Thirty-two years after the publication of the legendary 'Rasch book' (Rasch, 1960), the rich literature on the Rasch model and its extensions was scattered in journals and many less accessible sources, including 'grey' literature. When asked by students or junior researchers for references to the Rasch model, it was a typical reaction on the part of the editors to state that it was difficult to name one, or just a few; actually, only a whole list of references differing in notation and level of formal abstraction seemed to meet the request in most cases. Therefore, in 1992 the editors decided to invite a number of outstanding authors in the field of Rasch modeling to contribute to a book presenting the current state of knowledge about Rasch models. The aim was not just to collect a number of papers on the subject, rather to produce a well-organized monograph. To this end, a workshop was held in Vienna from 25 to 27 February 1993 in which, after a process of mutual reviewing, drafts of all chapters were read and discussed by all authors, leading to a more systematic organization of the topics treated in unified notation and terminology. (The workshop was sponsored by the University of Vienna; here, the editors would like to express their thanks, in the name of all contributors, both for the financial support and for the hospitality granted.

This unique collection of chapters from world experts on person-centered outcome (PCO) measures addresses the following critical questions: Can individual experiences be represented in measurements that do not reduce unique differences to meaningless uniformity? How person-centric are PCO measures? Are PCO measurements capable of delivering the kind of quality assured quantification required for high-stakes decision making? Are PCO measures likely to support improved health care delivery? Have pivotal clinical studies failed to deliver treatments for diseases because of shortcomings in the PCO measures used? Or is the lack of common languages for communicating outcomes also debilitating to quality improvement, research, and the health care economy? Three key issues form an urgent basis for further investigation. First, the numbers generated by PCO measures are increasingly used as the central dependent variables upon which high stakes decisions are made. The rising profile of PCO measures places new demands for higher quality information from scale and test construction, evaluation, selection, and interpretation. Second, PCO measurement science has well-established lessons to be learned from those who have built and established the science over many decades. Finally, the goal in making a PCO measurement is to inform outcome management. As such, it is vitally important that key stakeholders understand that, over the last half century, developments in psychometrics have refocused measurement on illuminating clinically important individual differences in the context of widely reproduced patterns of variation in health and functioning, comparable scale values for quality improvement, and practical explanatory models. This book’s audience includes anyone interested in person-centered care, including healthcare researchers and practitioners, policy makers, pharmaceutical industry representatives, clinicians, patient advocates, and metrologists. In their preface to the second edition of Test Equating, Scaling, and Linking, Mike Kolen and Bob Brennan (2004) made the following observation: “Prior to 1980, the subject of equating was ignored by most people in the measurement community except for psychometricians, who had responsibility for equating” (p. vii). The authors went on to say that considerably more attention is now paid to equating, indeed to all forms of linkages between tests, and that this increased attention can be attributed to several factors: 1. An increase in the number and variety of testing programs that use multiple forms and the recognition among professionals that these multiple forms need to be linked. 2. Test developers and publishers, in response to critics, often refer to the role of linking in reporting scores. 3. The accountability movement and fairness issues related to assessment have become much more visible. Those of us who work in this field know that ensuring comparability of scores is not an easy thing to do. Nonetheless, our customers—the te- takers and score users—either assume that scores on different forms of an assessment can be used interchangeably or, like the critics above, ask us to justify our comparability assumptions. And they are right to do this. After all, the test scores that we provide have an impact on decisions that affect people’s choices and their future plans. From an ethical point of view, we are obligated to get it right.

The goal of this book is to emphasize the formal statistical features of the practice of equating, linking, and scaling.
