For several decades the unsolved etiogenetic and therapeutic problems of multiple sclerosis have offered the strongest challenge to research in neu rology. The hope of decisive theoretical and practical progress increased when an experimental model presenting far-reaching conformity of structural and pathogenetic features was developed, namely chronic re lapsing experimental allergic encephalomyelitis (CREAE). During the past years, Dr. Lassmann has contributed substantially to the adaptation of this model with the aim of comprehensive evaluation, thoroughly fol lowing up his own ideas in numerous studies of individual aspects. The new possibility of continuous and detailed investigation of the clinical, morphological and immunological characteristics of temporal phase sequence of autoimmune demyelination has led to many new findings, corrections offormer hypotheses, and, from correlated studies of human multiple sclerosis, a series of important data concerning, for example, early manifestations of demyelination, the range of so-called acute mul tiple sclerosis and the incidence of remyelination. Moreover, Dr. Lass mann has analysed several special problems which became definable in the course of his own studies or in collaboration with other groups, in cluding the initial distribution of demyelinated foci, the cerebrospinal fluid phenomena and immunological findings in the nervous tissue. The results of these separate studies also led to a deeper understanding of demyelinating processes. This monograph integrates these studies and summarizes their re sults.

This book is based on the papers delivered at an International Workshop on Multiple Sclerosis held in Erice, Italy from 29th August to 1st September 1983. The Meeting was organized with great efficiency in the delightful setting of the Centro di Cultura Scientifica Ettore Mahorana, by its Director, Antonino Zichichi, and was directed by the Editors of the Proceedings and by Professor C. Alvisi of the University of Bologna. The emphasis was deliberately on the contributions of laboratory science to the understanding of mUltiple sclerosis, its etiology, pathogenesis, diagnosis and treatment. Where so much is unknown, disagreement was expected and indeed welcome, and it is regrettable that it was not possible to publish verbatim the often animated and always interesting discussion. Differing views on the relevance of experimental allergic encephalomyelitis to multiple sclerosis were expressed by Dr. Wisniewski, Professor Seitelberger and Professor Alvord, who graphi cally illustrated his changing beliefs over the years. Professor Seitelberger laid much greater emphasis on remyelination in remission in mUltiple sclerosis than had previously been the accepted view. The disorder of both humoral and cellular immunity in multiple scler osis was discussed by Dr. Roos and Professor Link. Dr. Gilden described how recombinative techniques might be applied to the detec tion of a virus in multiple sclerosis. The continued inability to detect any antigen to match the oligoclonal IgG bands in the CSF in multiple sclerosis patients was disappointing and there was an increasing tendency to regard the bands as "nonsense" antibodies.

The present book is based on the papers presented during an inter disciplinary meeting which was held in Rome October 27-30, 1986, at the Istituto Superiore di Sanita under the sponsorship of NATO, and with a financial contribution of the Cassa di Risparmio di Roma. The meeting was of great interest because, for the first time, a group of highly qualified scientists specializing in different fields were gathered together to discuss the problem of demyelinating diseases. In thi. context molecular biologists, immunologists, virologists, and lipid chemists could discuss with clinicians for four days every aspect of demyelinating diseases. They shared results of the more recent research with the hope that more knowledge of the myelinating process will eventually succeed in finding new treatments for these diseases. The meeting was successful, perhaps in part because discussions during lunches and dinners provided an opportunity for the exchange of new ideas. This was apparent in the discussions held after the presen tation of each paper and was summarized in the very lively round table which was held on the last day. The editor wishes to express his thanks to the scientific committee members: N. Baumann, C. Fieschi, G. Macchi, J.M. Matthieu, K. Suzuki. The present report, compares two murine models of virus induced chronic relapsing demyelination. MHV-induced demyelination in the BALB/c mouse results from the direct virus mediated cytolysis of oligodendrocytes. Extensive remyelination by oligodendrocytes is noted. Remyelination by oligo dendrocytes is delayed and incomplete. Chronic demyelination is widespread and associated with perivascular inflammatory infiltrates. The virus persists and viral antigen is local ized within oligodendrocytes.
