The complex challenges highlighted by the COVID-19 pandemic and other major health emergencies emphasize the need to rethink our approach to surveillance, while building upon the momentum of substantive investments in public health capacity in recent years. At the 75th World Health Assembly in May 2022, WHO set out a harmonizing framework to strengthen the global architecture for health emergency preparedness, response, and resilience (HEPR). Under the proposed global architecture, the ability to effectively prevent, prepare for, detect, respond to, and recover from health emergencies at subnational, national, regional and global levels depend on the operational readiness and capacities in five interconnected systems: collaborative surveillance, community protection, safe and scalable clinical care, access to countermeasures, and emergency coordination. This document defines the collaborative surveillance concept—proposing a conceptual model, dimensions across which collaboration should occur to enable multi-source and multisectoral surveillance, key objectives and concrete capabilities for how countries, with the support of WHO and partners, can further advance surveillance capabilities, and address fragmented and insufficient capacity. The collaborative surveillance concept was developed to support all stakeholders working on surveillance.

The WHO Benchmarks for International Health Regulations (2005) (IHR) Capacities was first published in 2019 and serves as a capacity-building tool and reference document to guide development/updating of country health security plans, including the national action plan for health security (NAPHS). It is now updated to a second edition which incorporates lessons learned from recent health emergencies, as well as alignment with updated IHRMEF tools, the HEPR framework, the WHO Director-General’s ten proposals to build a safer world together, and to build back better through multi-hazard and whole-of-society approaches to support better preparedness for future emergencies. Over 250 relevant technical leads contributed to this edition, by providing inputs from WHO regional offices, countries, partners and participation in global consultation meetings. The second edition is titled “WHO Benchmarks for Strengthening Health Emergency Capacities: Support for the Implementation of International Health Regulations (IHR) and Health Emergency Prevention, Preparedness, Response and Resilience (HEPR) Capacities”. WHO benchmarks are further digitalized for easy and quick use, along with a reference library, which is currently being updated. The audience for this document includes WHO Member States, health ministries and other relevant ministries, healthstakeholders, partners, nongovernmental organizations and academia to support building capacities at the country level.

Integrated surveillance in the context of the GISRS refers to expanding GISRS functions to include non-influenza respiratory viruses with epidemic or pandemic potential in order to inform public health interventions. Integrated surveillance encompasses the whole process, from the systematic screening of individuals, collection of respiratory specimens, testing, sequencing, data and information sharing and analysis. This document provides guidance on the surveillance standards and operational approaches, as well as practical considerations for the integration of SARS-CoV-2 and RSV surveillance based on standards developed for influenza, essential for an expanded GISRS that aims to strengthen national sentinel surveillance capacities for respiratory viruses as part of the mosaic of surveillance systems in a country, and in line with the overall global architecture of health emergency preparedness, response and resilience for strong national integrated disease, threat and vulnerability surveillance. This updated guidance stands in as the new guidance and standards for integrated surveillance, superseding these prior guidance documents: the End-to-end integration of SARS-CoV-2 and influenza sentinel surveillance: revised interim guidance, the Global epidemiological surveillance standards for influenza, and the WHO strategy for global Respiratory Syncytial Virus Surveillance project based on the influenza platform.

The Strategic Framework for enhancing control and achieving elimination of human-to[1]human transmission of mpox (2024-2027) is a guide to Member States on the next phase of the global mpox response and to enhance preparedness for future outbreaks. During the public health emergency of international concern (PHEIC) for the global mpox outbreak (July 2022-May 2023), the WHO supported Member States in taking action to stop the outbreak of mpox. In the transition from emergency to long-term response, WHO now recommends that all countries develop and implement plans for mpox control and the elimination of person-to-person transmission of this infectious disease, while continuing to ensure mitigation of zoonotic transmission. This strategic framework supports the development of these strategies.

Robust evidence from high-quality research is vital for pandemic and epidemic intelligence, forming the basis for effective collaborative surveillance and informed public health decisions. The COVID-19 pandemic has spurred innovation in data and laboratory science and improved our ability to detect, assess and respond to health threats. However, research on surveillance methods and tools requires greater coherence and more efficient ways to translate research for evidence-based policy-making. The WHO Pandemic Hub has coordinated a process to set research priorities that should be particularly relevant for routine public health surveillance practices in collaboration with the Global Research Collaboration for Infectious Disease Preparedness (GloPID-R), Charité University Hospital Berlin and the WHO Science Division and with a financial contribution and observer role of Wellcome Trust. Dieses Dokument definiert das Konzept von Collaborative und schlägt ein konzeptionelles Modell vor, ferner die Dimensionen, in denen die Zusammenarbeit stattfinden sollte, um die Überwachung von Mehrfachquellen und sektorübergreifend zu ermöglichen, sowie die zentralen Ziele und konkreten Fähigkeiten in Bezug auf die Frage, wie die Länder mit Unterstützung der WHO und ihrer Partner die Überwachungskapazitäten weiter ausbauen und dem Problem fragmentierter und unzureichender Kapazitäten entgegenwirken können.
