Alleine die quantitative Zunahme an kranken Patienten führt zu höheren Kosten. Diese müssen von der arbeitstätigen Bevölkerung getragen werden, welche in Zukunft relativ zu den Älteren betrachtet weiter abnehmen wird. Da die Finanzierung somit absehbar nicht dauerhaft und nachhaltig möglich sein wird, hat der Gesetzgeber das deutsche Gesundheitssystem in den letzten Jahren mehrfach reformiert. Ein Kernelement dieser Reformen ist die Implementierung von kosten- und qualitätsorientierten Markt- und Wettbewerbsprozessen in das Gesundheitssystem. Da jedoch wesentliche Bedingungen für einen funktionierenden Markt im wirtschaftswissenschaftlichen Sinne innerhalb des Gesundheitssystems nicht erfüllt sind und teilweise auch gar nicht erfüllbar sind, ist eine direkte Übertragung eines klassischen marktwirtschaftlichen Systems in das Gesundheitssystem mit den Gefahren des Marktversagens behaftet. Vor dem Hintergrund der nachhaltigen Finanzierungsproblematik infolge des demografischen Wandels sollen diese Besonderheiten und insbesondere ihre Auswirkungen und ihre Funktionsfähigkeit im Hinblick auf zukünftige Kernaufgaben des Gesundheitswesens analysiert werden. Teilaspekte der in Deutschland in Kraft getretenen Reformen, und nicht zuletzt das Beispiel der Niederlande, zeigen jedoch, dass marktwirtschaftliche Strukturen im Gesundheitswesen prinzipiell funktionieren können. Diese Strukturen müssen durch den Staat jedoch so reglementiert werden, dass keine Wirkungsmechanismen eines vollkommenen Marktes dominieren, d.h., dass neben zweifellos für eine nachhaltige Finanzierung des Gesamtsystems wichtigen Aspekten der Effizienz auch die Qualität und der gleiche Zugang zu notwendigen, qualitativ hochwertigen Gesundhei

This book highlights the reasons for an urgently needed revision of the current global healthcare setup, discusses the needed mindset for a future of health, and provides a comprehensive development toolset for disruption (and for the needed incremental innovations towards disruption). Today’s biomedical and health innovation related research in universities encourages activities that lead to incremental innovations with a relatively low risk of failure. The healthcare industry on the other hand provides tools and devices for established healthcare providers to improve the diagnosis and therapy/ treatment of the patients’ health problems. The patient is not in the center of healthcare provision however, and prevention and prediction are not core goals. The current health setup needs to be challenged and disrupted. Disruptions are coming from technologies or processes that lead to a significant (>10x) reduction in cost or price/ performance and that also come with new business models. The need for change, effects of exponential technologies, and the needed shift to prevention and to homecare for health democratization and patient empowerment will be discussed in detail in the first parts of the book. The subsequent sections address several innovation methods with a focus on a novel meta methodology named Purpose Launchpad Health. This is followed by a comprehensive discussion on health entrepreneurship activities and needs. The final section of the book addresses how to train students to become entrepreneurial health innovators, presenting successful curricula and examples of health incubation and accelerator setups. All of the innovation tools presented and used in this book are summarized in the final chapter to help the reader get started planning an entrepreneurial venture. In this work, novel contributions towards the emerging field of Ambient Assisted Living (AAL) are introduced. AAL is a concept envisioned in the early 2000s by the European Commission, aiming at supporting specifically senior people by means of technology and thus helping them to lead independent and self-determined lives in their accustomed surroundings as long as possible. Modern home automation technology is believed to be the key to providing various services in the fields of health, safety, comfort, and communication. In the framework of this thesis, health monitoring aspects are of particular interest. Inactivity monitoring is a very promising approach thereto since it allows the detection of potential health threats or cases of emergency without being overly privacy intrusive. Deriving condensed and dependable inactivity profiles representing typical user behaviour is a pivotal prerequisite for automatic emergency monitoring. Several methodologies for computing such patterns are introduced. Based on those inactivity profiles, various alarming criteria (i.e., permissible inactivity thresholds) are utilised to trigger alarms automatically if the users' inactivity levels exceed individual, user-dependant limits. Since false alarms are inevitable in automatic alarming systems, a procedure of handling them is introduced as well. Finally, the real-world application of the devised AAL system is illustrated.
