Health care-associated infections (HAI) are one of the most common adverse events in care delivery and a major public health problem with an impact on morbidity, mortality and quality of life. At any one time, up to 7% of patients in developed and 10% in developing countries will acquire at least one HAI. These infections also present a significant economic burden at the societal level. However, a large percentage are preventable through effective infection prevention and control (IPC) measures. These new guidelines on the core components of IPC programmes at the national and facility level will enhance the capacity of Member States to develop and implement effective technical and behaviour modifying interventions. They form a key part of WHO strategies to prevent current and future threats from infectious diseases such as Ebola, strengthen health service resilience, help combat antimicrobial resistance (AMR) and improve the overall quality of health care delivery. They are also intended to support countries in the development of their own national protocols for IPC and AMR action plans and to support health care facilities as they develop or strengthen their own approaches to IPC. These are the first international evidence-based guidelines on the core components of IPC programmes. These new WHO guidelines are applicable for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.

The Democratic Socialist Republic of Sri Lanka has a long and impressive history of strong leadership and commitment to International Health Regulations (IHR) (2005) compliance and health security. Health security in Sri Lanka dates back to 1897 when the Quarantine and Prevention of Diseases (QPD) Ordinance was enacted. Since the implementation of the IHR (2005) in 2007, Sri Lanka has made steady progress in implementing and developing IHR core capacities. In June 2017, Sri Lanka became one of the first countries in South-East Asia to conduct a Joint External Evaluation for IHR (JEE-IHR), using JEE Tool version 1, and prepare a National Action Plan for Health Security (NAPHS) for the five-year period of 2019–2023. In addition, Sri Lanka completed an IHR-Performance of Veterinary Services (PVS) National Bridging Workshop in February 2023, where the country’s human and animal health services reviewed their current collaboration strengths and gaps in key technical areas and developed a joint road map to improve work and coordination at the animal–human interface in the prevention, detection and control of zoonotic diseases. Sri Lanka also conducted a national workshop on strategic assessment of emergency risks using the Strategic Tool to Assess Risk in May 2023 to derive a national emergency risk profile, annual risk calendar, and recommendations to select and order the capacity-building interventions to be included in the next NAPHS, based on the priority actions stemming from this second JEE exercise.

The JEE team extends its sincere appreciation to the Republic of Iraq for volunteering for a second round of the JEE, and for being the second country in the WHO Eastern Mediterranean Region to complete this evaluation using the latest, third edition of the JEE tool. This revised tool incorporates lessons from the COVID-19 pandemic and other public health emergencies, ensuring a comprehensive assessment of health security capacities. Iraq’s commitment to health security is evident through its proactive participation in health evaluations. The country conducted its first JEE in 2019, followed by a Universal Health and Preparedness Review (UHPR) between December 2021 and March 2022. As the first Eastern Mediterranean country and the second globally to pilot the UHPR, Iraq assessed its national preparedness capacities using a health systems approach, leading to a comprehensive report endorsed by the Prime Minister in March 2022. The UHPR report, published online in August 2023, provides high-priority recommendations that complement the JEE findings.

Pandemic prevention and preparedness are at the top of everyone’s mind as the world continues to tackle the COVID-19 pandemic. During this time, however, the threats posed by the silent pandemic of antimicrobial resistance to human, animal, and plant health, food production, and the environment have not ceased and are steadily growing. More than ever before, we know that addressing the interlinked and multi-faceted challenges posed by antimicrobial resistance requires that we work together – across sectors, government, academic disciplines, civil society, the private sector, and the multilateral system – to advance a One Health approach. This strategic framework on antimicrobial resistance represents an important milestone in the decades-long history of collaboration between the Tripartite organizations - a collaboration that now is even stronger as a result of our close engagement with the United Nations Environment Programme. The framework sets out for the first time what our organizations – as leaders in the multilateral system on the human, animal, plant, and environmental health – will do jointly to support countries’ efforts to scale up national responses to antimicrobial resistance. Countries and partners are strongly encouraged to replicate and amplify the One Health approach used in the framework, based on their own contexts and needs.
