Diagnostic Surgical Neuropathology by Location, is a practical guide for all pathologists in their initial, real-time encounters with brain of spinal cord biopsies for the immediate rendering of an intraoperative diagnosis based on the squash preparations of frozen section. The book is organized with a heavy emphasis on neuroanatomic to neuropathologic relationships best defined as a given lesion's location with regard to its contiguous macroscopic and microscopic environments. The demonstration of the lesion's macroscopic environment would include corresponding radiographic images, diagrams, and photographs of applicable autopsy teaching specimens for comparison; the microscopic environment would be illustrated by photomicrographs expressly composed in ways to show lesions in relation to contiguous unaffected, non-diseased tissue. The chapters progress from the cranial-to-caudal segments of the neuraxis from brain to spinal cord, respectively. The topics to be included in this book consist of all lesions likely to be encountered in the course of intraoperative neuropathologic consultation; this would encompass both primary and metastatic (secondary) forms of CNS neoplasia, non-neoplastic CNS lesions (for example, demyelinative processes), and CNS infections. Not another textbook for neuropathology! Yes, Our publishers helped us with this last problem by we hear you and feel your pain. In fact, that was our insisting on black and white photographs. We ini initial response when we were approached to write tially protested, noting that many recent textbook the book you are now holding. In surveying the reviews seemed to be primarily guided by whether expanse of currently available neuropathology illustrations were in color (good) or black and white textbooks, however, we came to feel that there was (bad). However, upon further reflection we accepted a place for a book that could combine our career this mandate as a blessing in disguise, allowing the experiences of trying to discern what is known (and reader to focus on differences in morphology, rather knowable) with the perennially proposed question, than tincture, as a guide to correct diagnosis. In fact, "What do we need to know?" Together we tried to one of us (M. e. ) has always been a fan of black and produce a book that would be practical, understand white photography, both in histologic atlases as well able, and to the point (minimizing reading time as in the immortal photographs of artists ranging from during intraoperative consultation). Every year dozens of physicians-in-training face, for the first time, the responsibil ity of examining and diagnosing central nervous system tumors or biopsies of the central nervous system, the peripheral nerves or muscles, whose surgical resection has been decided on both as a form of treatment (in the case of tumors) and as means to confirm a presumptive diagnosis. The selection of the most appropriate form of post surgical treatment for most tumors is predicated on the precise identification of the tumor cells. The evaluation of the specimen, by a pathologist, will not only determine whether the lesion is truly neoplastic, but also whether there are histologic indicators of malig nancy. Moreover, in some cases, the pathologist will be asked to determine whether the tumor cells contain certain hormone precursors or receptors, as an example. Recognition of many of the features that one must search for requires the judicious application of methods that may not be readily known to the physicians involved in the various diagnostic procedures. The handling and processing of the tissues as they arrive in the pathology laboratory for the above reasons vary as a function of the organ (or site) of origin of a given tumor as well as a function of the presumptive clinical diagnosis. The material contained in this book series has been organized in an attempt to help the pathologists-in-training, the general pathologists, the neurosurgeons, and neu rologists to understand the logic behind such special requirements.
