Die unterschiedliche Gewichtung dieser Werte hat zu unterschiedlichen Entwicklungen und Herangehensweisen im Gesundheitswesen geführt. Unter Elemente, die für die Beschreibung und Evaluation von Gesundheitswesen von Bedeutung sind, fallen die gesundheitliche Versorgung durch Organisation und Bereitstellung von Gesundheitsdiensten, die Regulierung bzw. Reformierung des Gesundheitswesens in Form von politischen Entscheidungen und Mechanismen, sowie die Finanzierung des Gesundheitswesens. Unter diesen Gesichtspunkten können dem Gesundheitswesen und dessen Politik bestimmte Modelle zugeordnet werden, die sich durch Organisation und Finanzierung von Gesundheitsleistungen als auch durch den staatlichen Regulierungs- und Reformierungsgrad abgrenzen lassen. Dadurch lassen sich drei idealtypische Ordnungen von Gesundheitssystemen differenzieren: Das Beveridge-Modell, ein egalitäres, steuerfinanziertes System mit überwiegend staatlicher Organisation und standardisierten Gesundheitsleistungen. Des Weiteren das Bismarck-Modell, ein bezeichnendes Sozialversicherungssystem, das sich durch staatlich vorgelegte Gesundheitsversorgung durch Pflichtbeiträge und Leistungserbringung durch Selbstverwaltungspartner mittels öffentlichem, gemeinnützigem und privatem Eigentum kennzeichnet. Und das libertäre Markt-Modell mit überwiegend privater Finanzierung und Leistungserbringung unter eng limitierter Staatsgarantie bzw. Kontrolle.

Global public goods (GPGs)--the economic term for a broad range of goods and services that benefit everyone, including stable climate, public health, and economic security--pose notable governance challenges. At the national level, public goods are often provided by government, but at the global level there is no established state-like entity to take charge of their provision. The complex nature of many GPGs poses additional problems of coordination, knowledge generation and the formation of citizen preferences. This book considers traditional public economy theory of public goods provision as oversimplified, because it is state centered and fiscally focused. It develops a multidisciplinary look at the challenges of understanding and designing appropriate governance regimes for different types of goods in such areas as the environment, food security, and development assistance. The chapter authors, all leading scholars in the field, explore the misalignment between existing GPG policies and actors' incentives and understandings. They analyze the complex impact of incentives, the involvement of stakeholders in collective decision making, and the specific coordination needed for the generation of knowledge. The book shows that governance of GPGs must be democratic, reflexive--emphasizing collective learning processes--and knowledge based in order to be effective.

In this unique and provocative contribution to the literatures of political science and social policy, ten leading experts question prevailing views that federalism always inhibits the growth of social solidarity. Their comparative study of the evolution of political institutions and welfare states in the six oldest federal states - Australia, Austria, Canada, Germany, Switzerland, the US - reveals that federalism can facilitate and impede social policy development. Development is contingent on several time-dependent factors, including degree of democratization, type of federalism, and the stage of welfare state development and early distribution of social policy responsibility. The reciprocal nature of the federalism-social policy relationship also becomes apparent: the authors identify a set of important bypass structures within federal systems that have resulted from welfare state growth. In an era of retrenchment and unravelling unitary states, this study suggests that federalism may actually protect the welfare state, and welfare states may enhance national integration.
