The relationship between metabolic dysfunctions and cancer development has been continuously evolving, informed by research efforts in the fields of epidemiology, endocrinology, cancer biology, chemistry, genomics, metabolomics among others. Metabolism and cancer can intersect at the physiological level, where metabolic disorders and cancer malignancies occur as comorbid or mutually-promoting conditions; or at the molecular level, where metabolic alterations regulate tumorigenesis and oncogenic/tumor-suppressive pathways govern the onset of metabolic syndromes. The increasingly complex spider web connecting metabolic dysfunctions and cancer development reveals both distinctive and shared mechanisms underlying different disease indications. It also points to unique opportunities, either through repurposing existing tools or discovering new strategies, to develop targeted therapies against a wide variety of metabolic and cancerous diseases. Chemotherapy is one of the major treatment options for cancer patients; however, the efficacy of chemotherapeutic management of cancer is severely limited by multidrug resistance, in that cancer cells become simultaneously resistant to many structurally and mechanistically unrelated drugs. In the past three decades, a number of mechanisms by which cancer cells acquire multidrug resistance have been discovered. In addition, the development of agents or strategies to overcome resistance has been the subject of intense study. This book contains comprehensive and up-to-date reviews of multidrug resistance mechanisms, from over-expression of ATP-binding cassette drug transporters such as P-glycoprotein, multidrug resistance-associated proteins, and breast cancer resistance p- tein to the drug ratio-dependent antagonism and the paradigm of cancer stem cells. More than 18 million people in the United States have diabetes mellitus, and about 90% of these have the type 2 form of the disease. In addition, between 17 and 40 million people have insulin resistance, impaired glucose tolerance, or the cluster of abnormalities referred to variably as the metabolic syndrome, the dysmetabolic syndrome, syndrome X, or the insulin resistance syndrome. In all of these disorders, a central component of the pathophysiology is insulin resistance, i.e., reduced responsiveness to insulin in tissues such as muscle, fat and liver. Insulin resistance is also closely linked to other common health problems, including obesity, polycystic ovarian disease, hyperlipidemia, hypertension, and atherosclerosis.
