Abstract: By international standards, and given its relatively low per capita income, Vietnam has achieved substantial reductions in, and low levels of, infant and under-five mortality. Wagstaff and Nguyen review existing evidence and provide new evidence on whether, under the economic liberalization program known as Doi Moi, this reduction in child mortality has been sustained. They conclude that it has, but that the gains have been concentrated among the better-off. As a result, socioeconomic inequalities in child survival are evident in Vietnam"a change from the early 1990s when none were apparent. The authors develop survival models to find the causes of this differential decline in child mortality, and conclude that a number of factors have been at work, including reductions among the poor (but not among the better-off) in coverage of health services and in women's educational attainment. They argue that if the experience of the late 1990s is a guide to the future, the lack of progress among the poor will jeopardize Vietnam's chances of achieving the international development goals for child mortality. The authors examine various policy scenarios, including expanding coverage of health services, water and sanitation, and find that such measures, while useful, will have only a limited effect on the mortality of poor children. They find that programs aimed at narrowing the gap between the poor and better-off may have large beneficial effects on the various determinants of child survival. The last 35 years or so have witnessed a dramatic shift in the demography of many developing countries. Before 1960, there were substantial improvements in life expectancy, but fertility declines were very rare. Few people used modern contraceptives, and couples had large families. Since 1960, however, fertility rates have fallen in virtually every major geographic region of the world, for almost all political, social, and economic groups. What factors are responsible for the sharp decline in fertility? What role do child survival programs or family programs play in fertility declines? Casual observation suggests that a decline in infant and child mortality is the most important cause, but there is surprisingly little hard evidence for this conclusion. The papers in this volume explore the theoretical, methodological, and empirical dimensions of the fertility-mortality relationship. It includes several detailed case studies based on contemporary data from developing countries and on historical data from Europe and the United States.

Van der Klaauw and Wang focus on infant and child mortality in rural areas of India. They construct a flexible duration model framework that allows for frailty at multiple levels and interactions between the child's age and individual socioeconomic, and environmental characteristics. The model is estimated using the 1998-99 wave of the Indian National Family and Health Survey. The estimated results show that socioeconomic and environmental characteristics have significantly different effects on mortality rates at different ages. These are particularly important immediately after birth. The authors use the estimated model for policy experiments. These indicate that child mortality can be reduced substantially, particularly by improving the education of women and reducing indoor air pollution caused by cooking fuels. In addition, providing access to electricity and sanitation facilities can reduce under-five-years mortality rates significantly. This paper--a product of the Environment Department--is part of a larger effort in the department to improve our understanding of environmental determinants of child mortality in rural India.

This paper on the social gains from female education is part of a series, prepared by the World Bank, on the benefits of improving opportunities for women. The paper suggests that expanding women's opportunities enhances their productivity and earning potential and thus contributes to better economic performance and poverty alleviation. Education raises the productivity and earnings of both men and women. Over time female education also contributes to slower population growth and healthier families. The Bank believes that in efforts to expand women's opportunities, priority should be given to education through the secondary level, reproductive health, agriculture, private entrepreneurship, and the wage labor force. This paper is concerned with the estimation of these social gains from female education at the secondary level. The paper examines the role of female education, measured by gross enrollment rates at the secondary level, relative to, and or in combination with, some health and family planning services that influence fertility and infant mortality. It uses reduced form estimation of the total fertility rate and infant mortality rate. The paper presents cross country regressions based on data drawn from 72 developing countries. The analysis in this paper generally shows that female secondary education, family planning, and health programs all affect fertility and mortality, and the effect of female secondary education appears to be very strong. Results suggest that family planning will reduce fertility more when combined with female education, especially in countries that now have low female secondary enrollment levels. (DK).
