La Historia de la locura en la época clásica efectuaba la arqueología de la línea divisoria en virtud de la cual se separa en nuestras sociedades al loco del no-loco. El relato finaliza con la medicalización de la locura a principios del siglo XIX. El curso que Michel Foucault consagra a finales de 1973 y principios de 1974 al «poder psiquiátrico» prosigue esta historia dando, sin embargo, un giro al proyecto: acomete la genealogía de la psiquiatría, de la forma propia de «poder-saber» que ella constituye. Para ello resulta imposible partir de un saber médico sobre la locura, pues éste se demuestra inoperante en la práctica. Únicamente puede darse cuenta de la producción de verdad psiquiátrica sobre la locura a partir de los dispositivos y técnicas de poder que organizan el tratamiento de los locos en el periodo que va de Pinel a Charcot. La psiquiatría no nace como consecuencia de un nuevo progreso del conocimiento sobre la locura, sino de los dispositivos disciplinarios en los que se organiza entonces el régimen impuesto a la locura. Michel Foucault se ha interrogado frecuentemente sobre la vanidad del discurso de los psiquiatras, que se presenta en la práctica contemporánea de los peritajes judiciales. Desde ese punto de vista, El poder psiquiátrico prosigue el proyecto de una historia de las «ciencias» humanas. El curso concluye a finales del siglo XIX en el momento en el que se produce la doble «despsiquiatrización» de la locura, dispersa entre el neurólogo y el psicoanalista. De este modo, El Poder psiquiátrico propone una genealogía de los movimientos antipsiquiátricos que tanto han marcado la década de 1960.

Só se pode dar ciência da veridicção psiquiátrica sobre a loucura a partir dos dispositivos e das técnicas de poder que organizam o tratamento dos loucos nesse período que vai de Pinel a Charcot. A psiquiatria não nasce como conseqüência de um novo progresso no conhecimento da loucura, mas dos dispositivos disciplinares nos quais se organiza então o regime imposto à loucura. O poder psiquiátrico dá continuidade ao projeto de uma história das 'ciências humanas'.
