Momentary mood states impact behavior, such that mood is linked to the initiation, maintenance, and intensity of behavior (Gendolla, 2000). Negative affect, in particular, is associated with eating psychopathology, increasing in the hours prior to and decreasing in the hours following binge eating (e.g., Berg et al., 2015, De Young et al., 2013). Escape theory, as applied to binge eating, posits that binge eating is motivated by a desire to alleviate experiences of aversive self-awareness by facilitating the process of cognitive narrowing (Heatherton & Baumeister, 1991; Blackburn, Johnston, Blampied, Popp, & Kallen, 2006). This theory may be particularly well suited for explaining how complex emotions, such as guilt and shame, serve to maintain eating disorder behaviors. Research is lacking within the field of eating disorders for distinguishing these two emotions. The current study aimed to distinguish shame and guilt as they relate to binge eating and purging. It was hypothesized that (1) shame will be positively associated with binge eating, (2) guilt will be positively associated with binge eating when accompanied by social anxiety, and (3) guilt will be positively associated with purging among individuals who binge eat. Undergraduates (N = 347) from the University of Wyoming completed self-report measures including indices of guilt- and shame-proneness, eating disorder attitudes and behaviors, social anxiety, state-level guilt and shame, and momentary urges to engage in eating disorder behaviors. Results demonstrate support for the first hypothesis at both trait- and state-levels, indicating that shame may be uniquely associated with binge eating; however, guilt was not found to be uniquely associated with binge eating. Hypotheses two and three were not supported. Results indicate a difference between shame and guilt as they relate to eating disorder behaviors. Future directions are discussed.

ABSTRACT: Heatherton and Baumeister (1991) proposed that individuals engage in binge eating in an attempt to escape aversive self-awareness (escape theory). The current study sought to test the escape theory prediction that individuals who experience a stressor and blame themselves will consume more food than individuals experience a stressor and blame another person. This hypothesis was tested in an experimental study among 155 female undergraduate students who experienced a stressor and were randomly assigned to one of three groups: an internal attribution group (blame stressor on self), external attribution group (blame stressor on another participant), and a control group (no one to blame the stressor on). Following the stressor, participants were given access to chocolate, and told to consume as much as they needed to complete a bogus taste test. Contrary to escape theory predictions, there were no statistically significant differences between groups in the amount of chocolate consumed. In an adequately powered study, the findings did not conform to escape theory prediction. Limitations of the study include use of a nonclinical population and access to only one type of food (chocolate).

Escape theory proposes that aversive self-awareness (ASA) arises when individuals are aware of their failure to achieve their own goals. This state is hypothesized to lead to cognitive narrowing, a state during which the individual pushes meaningful thought out of awareness, inducing a state of reduced affect and present-focused awareness of immediate surroundings. Cognitive narrowing has been shown to lead to disinhibited behavior, such as loss of control (LOC) eating. This study was the first to test escape theory of LOC eating using ecological momentary assessment to measure ASA and cognitive narrowing as they naturalistically occur. Results showed that ASA predicted cognitive narrowing when assessments occurred in close temporal succession, but not when assessments were spaced further apart, suggesting the relationship between ASA and cognitive narrowing is very momentary. Cognitive narrowing also predicted LOC eating. However, the relationship between ASA and LOC eating was not fully mediated by cognitive narrowing, suggesting that cognitive narrowing is not necessary for LOC eating to occur. Furthermore, cognitive narrowing was not associated with an emotionally blunted experience. Instead, happiness and anxiety decreased, guilt and sadness increased, and anger and stress remained unchanged. After LOC eating, mood did not improve, but ASA decreased. Overall, results suggest that LOC eating occurs in response to ASA and cognitive narrowing, but the state of cognitive narrowing is not necessary to engage in LOC eating. These results highlight escape theory’s unique construct of ASA as an important predictor and intervention target in future studies, but reject the theory’s principle tenet that cognitive narrowing is a necessary state for LOC eating to occur.

Material Discourses of Health and Illness applies discursive approaches to the field of health psychology, in stark contrast to the bio-medical model of health and illness. The discursive approach uses the person's experience and feelings as the central focus of interest, whereas the more traditional models regarded these as coincidental and relatively unimportant. The book provides an accessible and compelling introduction to social constructionist and discursive approaches to those with limited previous knowledge of socio-linguistic theory and research. It provides practical examples of how these approaches can be applied to the field of health psychology with a collection of sophisticated discursive analyses which demonstrate the distinctive contribution that can be made by psychologists to a field that has been largely dominated by sociologists and anthropologists.
