This book studies the relationship between institutionalism and schizophrenia in the lives of mental patients. The authors observed schizophrenic patients in three different mental hospitals over a period of eight years. Their conclusions are important for the better management of institutions and for the future of extra-mural mental health services. The lives of long-term schizophrenic patients are strictly limited by their institutionalised environments, which often produce negative effects. For example, patients are especially vulnerable to social understimulation, reacing with apathy and withdrawal. On the positive side, symptoms such as delusions and hallucinations may actually decrease during institutionalisation. The interesting approach to the positive and negative effects of institutionalisation on schizophrenics will give this book a wide readership in psychiatry, social psychology and the social sciences as well as among social workers, nurses and occupational therapists.

The revisions of both DSM-IV and ICD-10 have again focused the interest of the field of psychiatry and clinical psychology on the issue of nosology. This interest has been further heightened by a series of controversies associated with the development of DSM-5 including the fate of proposed revisions of the personality disorders, bereavement, and the autism spectrum. Major debate arose within the DSM process about the criteria for changing criteria, leading to the creation of first the Scientific Review Committee and then a series of other oversight committees which weighed in on the final debates on the most controversial proposed additions to DSM-5, providing important influences on the final decisions. Contained within these debates were a range of conceptual and philosophical issues. Some of these - such as the definition of mental disorder or the problems of psychiatric " - have been with the field for a long time. Others - the concept of epistemic iteration as a framework for the introduction of nosologic change - are quite new. This book reviews issues within psychiatric nosology from clinical, historical and particularly philosophical perspectives. The book brings together a range of distinguished authors - including major psychiatric researchers, clinicians, historians and especially nosologists - including several leaders of the DSM-5 effort and the DSM Steering Committee. It also includes contributions from psychologists with a special interest in psychiatric nosology and philosophers with a wide range of orientations. The book is organized into four major sections: The first explores the nature of psychiatric illness and the way in which it is defined, including clinical and psychometric perspectives. The second section examines problems in the reification of psychiatric diagnostic criteria, the problem of psychiatric epidemics, and the nature and definition of individual symptoms. An innovative and integrative avenue toward understanding and treating mental health disorders Psychoimmunology is a rapidly maturing area of scientific endeavor that provides a compelling integrative link between the immune system and its response to stress and psychiatric illness. Stress initiates pathological changes by activating the immune and endocrine systems. Inflammation is at the core of the complex and interactive systems that both contribute to and result from psychopathology. Consequently, inflammation research advances our knowledge of the pathology of depression, schizophrenia, chronic fatigue syndrome, posttraumatic stress disorder and a host of co- morbid conditions, notably diabetes, cardiovascular disease and cerebrovascular disease. The sociology of science is dominated today by relativists who boldly argue that the content of science is not influenced by evidence from the empirical world but is instead socially constructed in the laboratory. Making Science is the first serious critique by a sociologist of the social constructivist position. Stephen Cole begins by making a distinction between two kinds of knowledge: the core, which consists of those contributions that have passed the test of evaluation and are universally accepted as true and important, and the research frontier, which is composed of all work in progress that is still under evaluation. Of the thousands of scientific contributions made each year, only a handful end up in the core. What distinguishes those that are successful? Agreeing with the constructivists, Cole argues that there exists no set of rules that enables scientists to certify the validity of frontier knowledge. This knowledge is "underdetermined" by the evidence, and therefore social factors--such as professional characteristics and intellectual authority--can and do play a crucial role in its evaluation. But Cole parts company with the constructivists when he asserts that it is impossible to understand which frontier knowledge wins a place in the core without first considering the cognitive characteristics of the contributions. He concludes that although the focus of scientific research, the rate of advance, and indeed the everyday making of science are influenced by social variables and processes, the content of the core of science is constrained by nature. In Making Science, Cole shows how social variables and cognitive variables interact in the evaluation of frontier knowledge.
