The biological and physical rationale for hadron therapy is well understood by the research community, but hadron therapy is not well established in mainstream medicine. This talk will describe the biological advantage of neutron therapy and the dose distribution advantage of proton therapy, followed by a discussion of the challenges to be met before hadron therapy can play a significant role in treating cancer. A proposal for a new research-oriented hadron clinic will be presented.

This brief provides an in-depth overview of the physics of hadron therapy, ranging from the history to the latest contributions to the subject. It covers the mechanisms of protons and carbon ions at the molecular level (DNA breaks and proteins 53BP1 and RPA), the physics and mathematics of accelerators (Cyclotron and Synchrotron), microdosimetry measurements (with new results so far achieved), and Monte Carlo simulations in hadron therapy using FLUKA (CERN) and MCHIT (FIAS) software. The text also includes information about proton therapy centers and carbon ion centers (PTCOG), as well as a comparison and discussion of both techniques in treatment planning and radiation monitoring. This brief is suitable for newcomers to medical physics as well as seasoned specialists in radiation oncology.

As proton therapy treatment centers become smaller and more cost-effective, education and training for today’s multi-disciplinary oncology teams are more important than ever before. This state-of-the-art reference brings you fully up to date with all aspects of proton therapy, with guidance you can trust from MD Anderson Cancer Center, the largest and most experienced proton therapy center in the world. Led by Drs. Steven J. Frank and W. Ronald Zhu, Proton Therapy provides a unique opportunity to benefit from the unsurpassed knowledge and expertise of an esteemed team of leaders in the field. Covers all cancers for which proton therapy is used most often, including prostate, head and neck, pediatrics, central nervous system, gastrointestinal, sarcomas, lungs, breast, lymphomas, and gynecologic cancers. Provides up-to-date information on radiobiology, treatment planning and quality assurance, indications for proton therapy, management approaches, and outcomes after proton therapy by disease site. Discusses technologic advances such as spot scanning and treatment planning systems for the management of solid tumors; radiobiology of proton therapy, including DNA damage and repair mechanisms and acute and late effects on normal tissues; and multifield optimized intensity-modulated proton therapy (MFO-IMPT) for optimizing the distribution of linear energy transfer (LET) of proton beams within target volumes and away from critical normal structures. Includes a special section on head and neck cases in the e-book that photographically illustrates the full cycle of proton therapy care.

Proton beam therapy is an emerging technology with promise of revolutionizing the treatment of cancer. While nearly half of all patients diagnosed with cancer in the US receive radiation therapy, the majority is delivered via electron accelerators, where photons are used to irradiate cancerous tissue. Because of the physical properties of photon beams, photons may deposit energy along their entire path length through the body. On the other hand, a proton beam directed at a tumor travels in a straight trajectory towards its target, gives off most of its energy at a defined depth called the Bragg peak, and then stops. While photons often deposit more energy within the healthy tissues of the body than within the cancer itself, protons can deposit most of their cancer-killing energy within the area of the tumor. As a result, in the properly selected patients, proton beam therapy has the ability to improve cure rates by increasing the dose delivered to the tumor and simultaneously reduce side-effects by decreasing the dose to surrounding tissue. The benefits of proton beam therapy in delivering a lethal hit to the target while sparing surrounding normal tissues from radiation are becoming applicable to an increasing number of patients and a growing list of conditions. In this book, the author will guide the reader through existing evidence supporting proton beam therapy for pediatric cancer, prostate cancer, lung cancer, brain tumors, spinal tumors, and several other conditions. The book will discuss which conditions are suitable for treatment with proton beam therapy, how the treatment is delivered, and the current data supporting its use.

Cancer Treatment, Radiation Oncology, PSI, Pencil Beam Scanning, History of Medicine Modern proton therapy is saving the lives of more and more children and adults with difficult-to-treat cancers. The spectacular rise of proton therapy might never have happened if it were not for the pioneering work of the Paul Scherrer Institute – a Swiss centre founded to explore physics and the nature of matter, but which turned its understanding of basic science into applications that would benefit mankind. This is the place that pioneered a technique of delivering protons to tumours called pencil beam scanning (also known as spot scanning) – now used in nearly every proton therapy centre in the world. It also developed techniques to more accurately deliver the correct proton dose to every part of a tumour – again, now used globally. This book is for anyone with an interest in scientists’ continual quest to find out more. It tells the remarkable story, spanning half a century, of the men and women at the Paul Scherrer Institute’s Center for Proton Therapy who had the knowledge, imagination and perseverance to bring their ideas to fruition. It demonstrates, perhaps most of all, just what can be achieved from close collaboration between physicians and scientists – bringing all the powers of physics to bear in the fight against cancer. "For a field to grow, and for the community of patients to be best served, it’s necessary to learn, grow, improve and share, the way it has been done at PSI. This book provides a look at that extremely important perspective. I, for one, read it with great pleasure." (Jay Flanz, Project Director and Technical Director of the Burr Proton Therapy Center, Massachusetts General Hospital)
