The immune system can deal effectively with the majority of viruses and bacteria, less effectively with parasites, and very poorly with cancer. Why is this so? Why are McFarlane Burnet's and Lewis Thomas' predictions that the immune system is in volved in ridding the body of cancer cells, encapsulated in the catchy phrase "immunologic surveillance," so difficult to experi mentally establish? Cancer differs from infectious agents in being derived from the host. Hence, it has been postulated that cancer cells lack anti gens that the immune system can recognize. They are not "im munogenic. " However, this argument is seriously weakened by the existence of numerous human autoimmune diseases, in which the immune system effectively recognizes and attacks a va riety of self tissues. Thus, the potential clearly exists for recogni tion of the surfaces of tumor cells. Professor Naor and his colleagues have written a book that explores another possible reason: cancer cells are recognized by the immune system-but is it possible that the consequence of recognition is inhibition of the immune system-by suppressor T cells or macrophages? The evolution of the malignant state may only occur in individuals who develop this suppression. This book reviews the evidence that suppressor cells, poorly characterized and difficult to study, may be of fundamental im portance in cancer. In fact, our incapacity to understand the na ture of suppressor cells and their mode of action is one of the ma jor problems in immunology research today.

Non-thermal irreversible electroporation is a new minimally invasive surgical p- cedure with unique molecular selectivity attributes – in fact it may be considered the first clinical molecular surgery procedure. This textbook focuses on the nascent field of Immunoepidemiology that addresses how differences in immune responses among individuals affect the epidemiology of infectious diseases, cancer, hypersensitivity, and autoimmunity. The idea for the book originated from a course entitled “Immunology for Epidemiologists“ at the Yale School of Public Health. While many fine textbooks are available that address the immunological responses of individuals to pathogens, these provided very little information regarding how immunological variation among populations affects the epidemiology of disease. Organ transplantation has been performed for almost 40 years with steadily increasing success regarding long-time survival of the graft, as well as quality of life for the patient. However, graft tolerance only can be achieved via induction and maintenance of an impaired immunological surveillance. An increase of skin cancers as a consequence of the lowered cellular immunoresponse seems to parallel the overall increased long-time survival rate of organ transplant recipients. Against the background of chronic immunosuppression known risk factors like the amount of sun exposure prior and post transplantation, oncogenic viruses as well as the genetic background, and place of residence (latitude) are strongly related with the increased skin cancer incidence. The increasing incidence of non-melanoma skin cancer, paralleling a prolonged survival of patients after organ transplantation, represents a significant reason for morbidity and long-term morbidity in organ transplant recipients worldwide. The incidence of non-melanoma skin cancer in liver-, kidney- and heart transplant recipients varies from 1.5 to 22 %, 2 to 24 % and 6 to 34 % after 5 years post transplant. Ultraviolet radiation as well as immunosuppressive therapy are crucial risk-factors regarding the induction and progression of skin cancer. Ultraviolet radiation is related to the induction of DNA damage, as well as interference with Langerhans cell antigen presentation and a Th1 – Th2 shift induced via a release of IL-10. Whereas the overall duration of immunosuppression and the cumulative dosage applied are relevant parameters in the evaluation of an increased tumor risk, individual differences between specific immunosuppressive agents remain unclear. The workup of genetic as well as other unclear phenomenon like the reverse BCC/SCC ratio might shed some further light into the genesis and immunology of the non-melanoma skin cancer in general.
