About 20% of people with epilepsy have seizures which are resistant to anticonvulsant medications. These drug-resistant seizures are called `intractable', and the patients who have them - about 1 in 500 of the general population - present a major challenge to neurologists and epilepsy associations. The present volume describes the symptomatology of the major `intractable' syndromes, the most appropriate drugs for each, and the possibilities for surgical control. Research related to the causes and effects of unchecked seizures is presented, and new directions in prevention and therapy are discussed.

Intractable Focal Epilepsyis a definitive multi author reference work covering the most difficult to treat epilepsies. The natural history, pathology and treatment, both medical and surgical, of the conditions underlying intractable seizures are fully described and illustrated. Including contributions from North America, Europe and the Pacific Rim, the emphasis throughout is on clinical diagnosis and treatment both medical and surgical, and their respective outcomes. The text is enhanced by over 250 illustrations, and the clear modern design enables the reader to assimilate information quickly. In addition to clinical treatment, there is a section on the costs of persistent intractable focal epilepsy and the costs of surgical treatment. Drs Oxbury, Polkey and Duchowny have produced a book that will appeal to epileptologists, paediatric neurologists, and neurosurgeons. The books clinical strengths will make it an invaluable working tool. Definitive resource on difficult to treat epilepsies Covers manifestation, consequences and treatment Major section on surgical treatment of focal epilepsy Complete coverage of paediatric diagnosis and treatment Special "Case Reports" give practical case histories to illustrate clinical points International line up of contributors

Various subcortical structures in the brain are thought to be important in genesis or propagation of seizures. Deep Brain Stimulation (DBS) of these structures aims to influence these key “pacemaker” areas in the brain and is gaining increasing interest as a possible new surgical modality for treatment of patients with intractable epilepsy. Based on this background, this volume brings together the experience of the Zurich Epilepsy Surgery Center with stereotactic recordings and stimulations of various subcortical nuclei, with special reference to the Forel H fields, the thalamic nuclei and the periaqueductal gray matter. This volume also addresses the important issues of DC recording/polarisation and of biochemical measurement of excitatory and inhibitory transmitters released in the primary epileptogenic area during epileptic discharges. This book is destined to improve understanding of the involvement of subcortical structures in seizure genesis or propagation in a time when exhaustive invasive presurgical evaluation is diminishing because of the today’s availability of modern non-invasive imaging technique.

There are numerous books dealing with epilepsy but none of them is entirely devoted to the manifold relationships between language and epilepsy. The present monograph focuses on the verbal shortcomings, disorders, deviances, and peculiarities which may be observed in patients with epilepsy, and endeavors to uncover the links between the observed linguistic features and the disturbance of bio-electrical activity that characterizes epilepsy. It describes the many verbal impairments and involuntary verbal behaviours which may occur during epileptic seizures, depicts the various types of language-induced reflex epilepsy, discusses the possible relations between stuttering and epilepsy and between literary production and epilepsy, analyses the verbal consequences of surgery for intractable epilepsy, and deals extensively with the durable verbal deficits which may be due to, or be concomitant with, some forms of abnormal bio-electrical brain activity. The book is meant primarily for clinicians and caretakers who are professionally concerned with epilepsy. However, it is written in such a way that it can be easily understood by lay people who would like to gain an insight into the nature of epilepsy and into the verbal phenomena which may accompany it.

Over 12 million people with epilepsy have seizures that cannot be controlled by antiepileptic drugs. The term "drug-resistance" is abundantly used in the epilepsy literature but the definitions proposed differ considerably. It is used indifferently in a number of different settings: as a criterion for selection of patients eligible for new antiepileptic drug trials, for the selection of surgical candidates, for the design of epidemiological studies, for the design of studies on quality of life, for the definition of the epileptic encephalopathies (in comparison to more benign epilepsy syndromes), to mention but a few. As a result, available studies are usually not comparable and referral to epilepsy specialists is unacceptably delayed. The volume includes several focused chapters on all issues relating to drug-resistance and offers the basis for a consensus on a clinically meaningful core definition.

In most patients with epilepsy the prognosis for seizure control is very good. However, despite the introduction of new antiepileptic drugs and excellent monitoring methods, about 25% of patients develop chronic epilepsy which does not respond to treatment. Intractable epilepsy has a major impact on patients' quality of life and represents a considerable challenge for both the clinician attempting to effect seizure control and the researcher trying to understand why it develops and whether it can be prevented. This important new book defines intractable epilepsy, examines its consequences, risks and costs, discusses diagnosis and medical management, including novel drug therapies, neurosurgery and vagal stimulation, and sheds new light on its evolution and aetiology. Intractable Epilepsy is intended for neurologists, paediatricians, neurosurgeons, psychiatrists and other professionals involved in the management, investigation or study of intractable seizures.
