Ischaemic preconditioning is a phenomenon where brief periods of ischaemia and reperfusion prior to a prolonged episode of ischaemia have a protective effect on tissue. Ischaemic preconditioning has been demonstrated in every animal species tested and many different tissues, including human myocardium (Yellon et al, 1993), (Hausenloy et al, 2007). Many different patterns of ischaemic preconditioning have been tested (Kloner and Jennings, 2001 a), and this investigation aimed to identify a pattern of ischaemic preconditioning that promoted protective effects in a rat heart strip model, with recovery of contractile function as end-point assessment. Three different patterns of ischaemia and reperfusion were tested and compared to a control group which received no preconditioning ischaemia. After preconditioning, all four groups were exposed to 30 minutes of ischaemia and 30 minutes reperfusion. Results showed some preconditioning effects in group 3 although statistical analysis found the results not to be significant. However, the pattern used in group 4 was found to exacerbate ischaemic damage, which was unexpected. Further testing is suggested.

The tenth Henry Goldberg Workshop is an excellent occasion to recall our goals and celebrate some of our humble achievements. The first Goldberg Workshop, held in Haifa, in 1984, explored the interaction of mechanics, electrical activation, perfusion and metabolism, emphasizing imaging in the clinical environment. The second Workshop, in 1985, discussed the same parameters with a slant towards the control aspects. The third Goldberg Workshop, held in the USA at Rutgers University, in 1986, highlighted the transformation of the microscale activation phenomena to macro scale activity and performance, relating electrophysiology, energy metabolism and cardiac mechanics. The fourth Goldberg Workshop continued the effort to elucidate the various parameters affecting cardiac performance, with emphasis on the ischemic heart. The fifth Workshop concentrated on the effect of the inhomogeneity of the cardiac muscle on its performance. The sixth Workshop highlighted new imaging techniques which allow insight into the local and global cardiac performance.

We have recently shown that there is a neonatal developmental change in the ability of the heart to be protected by ischemic preconditioning (IPC) which matures over the first 21 days in the rat and the first year of life in the human. While there is much controversy surrounding the exact signalling pathway involved in preconditioning, evidence suggests it involves - activation of G-protein coupled receptors -protein kinase activation - mitochondrial KATP (MITOKATP) channel opening - mitochondrial depolarisation - reactive oxygen species generation - inhibition of the mitochondrial permeability transition pore (MPTP) leading to cardioprotection. The aim of these studies was to establish which part(s) of this putative cascade is/are immature in human and rat neonatal hearts. -- Human right atrial tissue was obtained during elective surgery for congenital defects and was used in Western blotting and contractility/cardioprotection studies (Note: no differences in atrial vs ventricular developmental changes in preconditioning have been previously observed). Background. Stem cell therapy and remote ischaemic preconditioning (RIPC) are emerging as potential strategies to protect the heart. Endothelial progenitor cells (EPCs) are a subset of stem cells which are thought to contribute to endothelial health, but their role in RIPC remains unclear. - Methods. An assay for the characterization of EPC's was developed using flow cytometry to enumerate CD133+/CD34+ cells. To validate this assay, EPC's were compared against endothelial function in a study comparing hypertensive individuals and healthy control subjects. The effect of intermittent limb ischaemia upon the warm-up angina phenomenon was then investigated, a potential human model of ischaemic preconditioning. This would determine whether the warm-up effect was affected by remote ischaemic preconditioning, abrogated by the free radical scavenger N-acetylcysteine (MAC), and whether EPC's correlated with changes observed. -- Results. A rapid, reproducible assay to enumerate EPCs was developed. EPCs appeared to correlate with hypertension, age and smoking in test subjects, but not with endothelial function as measured by flow-mediated dilatation (FMD). No correlation was found between EPCs and the protective effects of warm up angina. However, RIPC appeared to confer benefit in patients who manifested warm up angina. Furthermore, MAC appeared to abrogate protection of warm up angina, but not in the presence of RIPC. -- Conclusions. EPCs may be a useful biomarker of stable cardiovascular disease, but appear not to correlate with more acute changes in myocardial ischaemia. RIPC may be beneficial to some patients with ischaemic heart disease who manifest the warm up angina phenomenon. MAC appeared to abrogate the warm up effect, but not the protection conferred by RIPC.
