Il volume raccoglie una serie di qualificati contributi sulle "buone pratiche" di promozione sociale delle persone con sofferenza mentale nei DSM e sullo stato di servizi per la tutela della salute mentale a Roma. I risultati ottenuti confermano la validità dei percorsi di riabilitazione e di inclusione sociale che riducono drasticamente il ricorso al ricovero e consentono il reinserimento sociale e lavorativo di gran parte delle persone correttamente prese in cura. L'insufficienza dei servizi territoriali, dovuta soprattutto a gravi carenze di personale, limita tuttavia l'intercettazione della sofferenza e l'accesso a tali percorsi favorendo l'abbandono e la cronicizzazione. Ampio spazio viene dato al punto di vista delle associazioni degli utenti dei servizi e degli organismi consultivi per la salute mentale della Città di Roma (Consulte dipartimentali, Consulta cittadina, Consulta regionale). Il quadro che ne emerge è quello paradossale ed inquietante del sistema di tuela della salute mentale a Roma e delle sue drammatiche contraddizioni: da una parte, una realtà molto avanzata che, dopo la chiusura del manicomio, ha costruito faticosamente un sistema di presa in carico e di promozione sociale delle personecon sofferenza mentale che ha dato e dà risultati eccellenti, dall'altra un grave ritardo delle Istituzioni che non hanno, finora, sostenuto adeguatamente la piena realizzazione di un tale modello.

L’assistenza psichiatrica in Italia ha lunga tradizione e grande visibilità anche internazionale a seguito della Legge 180 del 1978 che, recepita dalla Legge 833/78, ha promosso l’abolizione degli ospedali psichiatrici e l’istituzione di Servizi Psichiatrici di Diagnosi e Cura (SPDC) quali unità organizzative specialistiche con un numero limitato di posti letto per il trattamento dei disturbi psichiatrici in fase acuta all’interno di ospedali. L’avvento del D.lgs 502/92 ha consentito ai Dipartimenti di Salute Mentale (DSM) di promuovere modelli si servizio nuovi in ragione dell’autonomia riconosciuta alle aziende sanitarie e ai responsabili di articolazioni organizzative. Inoltre, il Progetto Obiettivo Nazionale “Tutela della Salute Mentale 1994-1996” ha indicato come priorità l’istituzione del DSM (DSM) in tutte le Aziende Sanitarie Locali (ASL) ponendo al centro dell’intervento psichiatrico il Centro di Salute Mentale (struttura sanitaria territoriale, non ospedaliera). Oggigiorno le limitate interdipendenze che i DSM hanno sviluppato con la filiera di servizi delle ASL, le scelte delle Regioni che ne disegnano i confini e le aree di intervento in modo diverso, associate al ridisegno degli assetti istituzionali e organizzativi delle aziende sanitarie mettono in discussione questa lunga storia e pongono alcuni quesiti quali: "è il momento di mettere in discussione il DSM, così come è stato interpretato nel recente passato?", "quali modelli di gestione e formule di servizio possono profilarsi all'interno di contesti aziendali sempre più vasti territorialmente?". Il presente contributo prova a dare risposta a questi e ad altri interrogativi attraverso l’analisi di molteplici casi aziendali di DSM e una survey condotta a livello nazionale.
