In 1978 Italy passed a deeply radical law closing all its mental health hospitals. This was the culmination of the growth and development of a very strong anti-psychiatry movement which had sprung up in the late 1960's. Both the law, the movement, and its aftermath have been much discussed in Britain, America and other European countries because of the need to reconsider their own mental health care policies, but up to now there has been a lack of reliable literature on which to base the discussion. The Politics of Mental Health in Italy provides for the first time a scholarly and very balanced account of events and phenomena that have been previously presented in a more idiosyncratic and polemical fashion. Michael Donnely introduces, documents and comments critically on the three phases of the Italian experience: the late sixties mental health movement; the drafting and passage of the 1978 law; and the aftermath of deinstitutionalisation, which has disappointed its supporters and kept the whole topic at the centre of public debate.

L’assistenza psichiatrica in Italia ha lunga tradizione e grande visibilità anche internazionale a seguito della Legge 180 del 1978 che, recepita dalla Legge 833/78, ha promosso l’abolizione degli ospedali psichiatrici e l’istituzione di Servizi Psichiatrici di Diagnosi e Cura (SPDC) quali unità organizzative specialistiche con un numero limitato di posti letto per il trattamento dei disturbi psichiatrici in fase acuta all’interno di ospedali. L’avvento del D.lgs 502/92 ha consentito ai Dipartimenti di Salute Mentale (DSM) di promuovere modelli si servizio nuovi in ragione dell’autonomia riconosciuta alle aziende sanitarie e ai responsabili di articolazioni organizzative. Inoltre, il Progetto Obiettivo Nazionale “Tutela della Salute Mentale 1994-1996” ha indicato come priorità l’istituzione del DSM (DSM) in tutte le Aziende Sanitarie Locali (ASL) ponendo al centro dell’intervento psichiatrico il Centro di Salute Mentale (struttura sanitaria territoriale, non ospedaliera). Oggigiorno le limitate interdipendenze che i DSM hanno sviluppato con la filiera di servizi delle ASL, le scelte delle Regioni che ne disegnano i confini e le aree di intervento in modo diverso, associate al ridisegno degli assetti istituzionali e organizzativi delle aziende sanitarie mettono in discussione questa lunga storia e pongono alcuni quesiti quali: "è il momento di mettere in discussione il DSM, così come è stato interpretato nel recente passato?", "quali modelli di gestione e formule di servizio possono profilarsi all'interno di contesti aziendali sempre più vasti territorialmente?". Il presente contributo prova a dare risposta a questi e ad altri interrogativi attraverso l’analisi di molteplici casi aziendali di DSM e una survey condotta a livello nazionale.

Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Basaglia's International Legacy: From Asylum to Community provides an overview of current thinking and the international influence of Franco Basaglia. The book presents the results of an ethnographic study examining the post-deinstitutionalized organization and provision of acute mental health care in Italy. While the achievements of the “Basaglia law” which imposed the closure of psychiatric hospitals in Italy in 1978 have been well-documented, this book sheds fresh light on its aftermath and possible continuing influence. The author examines two Italian regions – Piedmont and Friuli Venezia Giulia (internationally known to be the first Italian region to close down asylums) – respectively as representatives of the ‘restraint’ and ‘no-restraint’ models. Within each context, participant observation and discursive interviews have been conducted in Mental Health Centres (CSM) and acute psychiatric wards (SPDC) to explore care and coercive practices, as well as notions of ‘good care’ and values embedded in everyday working activities of these services. Situated suggestions for possible improvement of today’s acute mental health care are also proposed. This book offers a novel ethnography of mental health care in the Italian context that will appeal in particular to practitioners and scholars in the fields of critical mental health, cross-cultural psychology, the history of psychiatry and the sociology of health.
