Vingt ans après leur fin abrupte, et malgré le caractère encore partiel de la réception de son travail (la moitié des leçons au Collège de France demeurent non publiées), les analyses du pouvoir élaborées par Michel Foucault (1926-1984) ont une influence durable sur un nombre impressionnant de sphères du savoir et de domaines de pratique. Foucault détruit le modèle classique du pouvoir conçu comme la possession d’une figure souveraine ou d’une classe sociale dominante. Il réforme les théories traditionnelles pour mieux rendre compte des techniques extra-institutionnelles de contrôle. Ces exercices de pouvoir immanents se manifestent sous des formes multiples et subtiles dans notre contexte « libéral » où la qualité des gouvernements se réduit le plus souvent à des capacités de gestion qui ouvrent sur une déresponsabilisation sociale grandissante. Foucault fait état de cette situation en analysant les formes variées du pouvoir qui s’exercent à travers des pratiques d’auto-gouvernance des individus et des populations. Les outils conceptuels qu’il développe (savoir/pouvoir, discipline, biopolitique, gouvernementalité...) permettent d’expliquer ces nouveaux modes de contrôle qui répondent à une logique inédite de décentralisation des pouvoirs étatiques en affectant toutes les conditions d’organisation de la vie dans nos sociétés (médicalisation, législation, criminalisation, judiciarisation, urbanisation...). Les contributions ici réunies rendent hommage à l’un des observateurs les plus éclairés des transformations de notre actualité. Elles ont été présentées à l’occasion d’un colloque international organisé à Montréal en mai 2004. Trois lettres accompagnent ce livre : l'une au président Jimmy Carter, ancien président des USA, qui a eu à gérer l'accident de Three Mile Island (TMI), l'autre au Pape François et enfin une troisième à la jeunesse engagée dans la lutte contre le changement climatique. Comment la nature construit-elle des ouragans ? Et peut-on apaiser ces structures auto-organisées ? On révèle aussi le moyen à utiliser pour calmer les phénomènes chaotiques qui ont envahi les soupapes de sécurité alors qu'elles sont chargées de protéger les populations, les installations et l'environnement. L'accident de Three Mile Island en est l'exemple type. C'est l'objet de la lettre ouverte envoyée au président Carter (sa réponse est jointe). Notre monde est un mélange subtil d'ordre et de désordre ; il est resté longtemps en partie incompréhensible. Les découvertes en physique au cours des deux derniers siècles ont conduit à des développements considérables, repris pour l'essentiel dans ce livre écrit avec le souci de la clarté. Ce texte rappelle les bouleversements successifs qui ont ébranlé la science et ont permis d'éclaircir les notions d'ordre et de désordre, de bien et de mal disent les philosophes, notions qui interpellent l'humanité depuis l'Antiquité. Dans les années 1970, la théorie du chaos bouscula notre vision déterministe du monde et les structures dissipatives de Prigogine mirent en évidence l'émergence d'ordre lorsqu'un système dissipatif est poussé loin de l'équilibre. Or certaines de ces structures dissipatives naissantes sont néfastes pour le genre humain, et peuvent conduire à des désastres. C'est ainsi qu'une tempête tropicale peut devenir un ouragan, c'est-à-dire un énorme moteur thermique mobile sur la surface de l'océan qui vient s'essouffler sur les terres habitées en y semant la désolation. On propose donc de détruire cette structure dissipative dangereuse en supprimant l'ordre qui s'est introduit lors de sa formation. Mais est-il permis d'éliminer de l'ordre dans la nature ? C'est l'objet de la note adressée au Pape François et aux autorités religieuses. Ainsi, dans ce livre, on s'intéresse au désordre complet, lorsqu'on supprime l'ordre dans une structure dissipative, avec applications aux soupapes des centrales nucléaires et aux ouragans. Et on évoque aussi l'ordre parfait, lorsqu'on élimine le désordre. On est alors tenu de faire référence aux descriptions disponibles dans les textes sacrés. Et on est surpris !

Seclusion as a concept is poorly understood and this is reflected in the literature on the topic, particularly from nursing authors. This has led to an emotionally charged altercation rather than academic debate, both within the literature and at conferences. But why bother learning about seclusion at all, particularly as it is used less and less within mental health? We would point out to those sceptical about the value of this book that seclusion is not only of interest as an intervention per se, but is valuable in reflecting a shifting ethos within care. For some reason, seclusion has been neglected; we believe that one reason is that it impinges upon widely held myths and beliefs within psychiatric practice. Questioning about seclusion uncovers uncomfortable facts and assumptions concerning the values underpinning today's mental health care approaches. Such uncomfortable questioning is often avoided for safer research pursuits. Also, we hold that this book is necessary in examining issues pertaining to seclusion practice. There is a gap within nursing knowledge in so far as seclusion is concerned, as our chapter on education upholds. Yet inquiries and litigation have highlighted the fact that seclusion practice must be more clearly understood as an intervention. At present, such understanding is erratic and far from useful in providing a higher standard of care. Practitioners need to make informed decisions regarding seclu sion, and this book aims to provide the necessary information on which to base these decisions.
