CONTEXTE La ponction lombaire (PL) est un acte invasif réalisé quotidiennement aux urgences. Soulager la douleur induite par les soins est une priorité pour le confort du patient. Le mélange équimolaire oxygène-protoxyde d'azote (MEOPA) est un gaz anxiolytique et antalgique avec une facilité d'utilisation intéressante en urgence. Démontrée en pédiatrie et pour la réalisation de PL programmées, son efficacité n'a jamais été étudiée aux urgences chez l'adulte. OBJECTIF Evaluer l'efficacité antalgique de l'administration de MEOPA lors de la réalisation de PL à visée diagnostique aux urgences adultes. METHODE Essai thérapeutique prospectif contrôlé randomisé en double aveugle à deux bras parallèles comparant le MEOPA à un placebo. Il a été réalisé dans le service des urgences du CHU de Clermont- Ferrand auprès d'adultes capables et naïfs de MEOPA ayant un IMC≤35. Un anesthésiant topique (EMLA) était appliqué. Le critère d'évaluation principal était le niveau de douleur maximal ressenti par le patient pendant la procédure. Le niveau d'anxiété maximal et de satisfaction globale étaient également étudiés. L'outil de mesure était une échelle numérique simple (0-10). RESULTATS L'analyse a été réalisée en intention de traiter sur 79 patients randomisés. La douleur maximale était de 4,9±3,0 pour les 38 patients recevant de l'air et de 4.0±3,0 pour les 41 recevant le MEOPA (P=0,202). L'anxiété maximale était 4,8±2,9 contre 3,6 ± 3,8 (P=0,115). La satisfaction globale était cependant de 8,9±1,6 dans le bras MEOPA contre 7,6±2,4 au sein du bras air (P=0.003). Il était nécessaire de traiter 5,46 patients pour éviter une douleur modérée (≥4/10) chez l'un d'entre eux. CONCLUSION Le MEOPA ne permet pas une diminution statistiquement significative du score de douleur et d'anxiété pendant la ponction lombaire aux urgences. Cependant il offre une amélioration de la satisfaction globale du patient et permet d'obtenir une douleur et une anxiété ≤4 chez plus de la moitié des patients traités contre un tiers sous placebo.
