No disease can exist by itself but rather each must be viewed as the result of some specific interaction between man and his environment. This fact is appreciated even by people in the most primitive of civilisations and prompts them to ask the basic question: 'Why did I develop this disease'? The astute clinician goes beyond establishing a diagnosis and asks similar questions of himself and his patient. Frequently a singular observation resulting from this type of inquiry provides the working hypothesis for the epidemiologist who again asks the same question, not of the individual but of a definable population at risk. As a science, epidemiology is bound to the laws of logic and statistics, but clearly it must go beyond this point. The epidemiologist faced with a statistic ally significant observation must also question" its biologic relevance. He must evaluate the consistency of his findings with the more estab lished features of the disease under study. He must also be sufficiently imaginative to challenge his results with additional studies, different in their methodologic approach; and, of great importance, he must have the courage to be wrong. But epidemiologic studies alone can never determine the specific aetiology of a disease. Other scientific disciplines with their own inherent limitations are also required. Thus, in cancer research, the laboratory investigator frequently employs animal models, but the applicability of results obtained to humans is always subject to question.

Until recently, understanding of the lymphomas was limited and largely descriptive. Attention has been focused, for the most part, upon morphological issues and clinical matters. Although useful, this approach has many shortcomings. The true cytoidentity of primary neoplastic cells was not established by these methods, nor could their clonal nature be recognized. The more overt changes in immunological function, such as monoclonal gammopathies and immunodeficiencies, were appre ciated as important components of these diseases. However, subtle immunological perturbations were not recognized. Furthermore, associations were not established between the lymphoreticular neoplasms on the one hand and both primary and secondary immunological abnormalities on the other. There has been considerable recent progress in the fields of immunobiology, cytology, and immunochemistry. These new approaches have proved readily applicable to studies on the lymphomas. The term "lymphoma" has been applied to a heterogeneous group of neo plasms that involve lymphoid tissues. This term is not altogether satisfactory, since it implies that cells of primarily the lymphoid series are involved in the neoplastic process. Some neoplasms, heretofore classified as "lymphomas," now appear, from the results of penetrating analysis using newer methodology, to be malignan cies of the macrophage series, rather than of the lymphocytic series. These neo plasms include Hodgkin's disease and a minority of neoplasms previously referred to as "histiocytic" lymphomas. The majority of these "histiocytic" lymphomas are now known to be lymphoid malignancies involving the B-cell series.

More than 150 years have passed since involvement of the nervous system in leukemia was first reported by BURNS, while the possibility of the primary brain tumor derived from the lymphoreticular tissue was recognized much later. BAILEY in 1929 described such neoplasms under the term perithelial sarcoma. Later these tumors of the nervous system have been variously designated as "Perivascular of perithelial sarcomas, reticulum cell sarcomas or microgliomas", as controversy has evolved and still exists on the definition of this group of neoplasms and on the nature of their cells of origin. Much of the unfortunate confusion concerning the neoplastic lesions attributed to "microglia" and other derivations of the RE system arose because their close relationship to extra neural cells has been ignored. On the other hand, communication in the field of malignant lymphomas has become increasingly difficult because of the termino logic maze and conceptual diversities of traditional morphological classifica tions that have limited valid comparisons. Although at present we are still far from a precise recognition of the basic features of lymphoproliferative processes and from a fairly general agreement on the terminology of non-Hodgkin lymphomas, there is encouraging evidence recently of some enlightment on the nature of tumor cells that provides the basis for a reasonable and generally acceptable lymphoma classification.

The geography of medicine is a classical subject that can be studied wherever medicine is practised and the population counted. It has already contributed a great deal to knowledge of the causes of disease and it might be thought that the subject had little more to teach. This, however, is unlikely. First, there are many parts of the world that have only recently been provided with the full range of diagnostic services. Secondly, it is now realised that the ob servation that a disease occurs only very rarely under some conditions can be just as important as the observation that it occurs very often under others. Thirdly, the spread of industrializa tion has not yet eliminated the wide variety of conditions of life that has been produced by different cultures and different standards of living. We have still an opportunity to determine their effect on the incidence of disease, but not perhaps for long. That an interest in the geography of cancer can pay dividends is shown by many examples ranging from the relationships between cancer of the buccal cavity and chewing habits and between cancer of the lung and smoking, to the current enquiries relating Burkitt's lymphoma to malaria, cancer of the oesophagus to the comsumption of maize beer, cancer of the liver to food contaminated with aflatoxin, and cancer of the large bowel to the dietary content of fibre and fat.
