Until recently, understanding of the lymphomas was limited and largely descriptive. Attention has been focused, for the most part, upon morphological issues and clinical matters. Although useful, this approach has many shortcomings. The true cytoidentity of primary neoplastic cells was not established by these methods, nor could their clonal nature be recognized. The more overt changes in immunological function, such as monoclonal gammopathies and immunodeficiencies, were appre ciated as important components of these diseases. However, subtle immunological perturbations were not recognized. Furthermore, associations were not established between the lymphoreticular neoplasms on the one hand and both primary and secondary immunological abnormalities on the other. There has been considerable recent progress in the fields of immunobiology, cytology, and immunochemistry. These new approaches have proved readily applicable to studies on the lymphomas. The term "lymphoma" has been applied to a heterogeneous group of neo plasms that involve lymphoid tissues. This term is not altogether satisfactory, since it implies that cells of primarily the lymphoid series are involved in the neoplastic process. Some neoplasms, heretofore classified as "lymphomas," now appear, from the results of penetrating analysis using newer methodology, to be malignan cies of the macrophage series, rather than of the lymphocytic series. These neo plasms include Hodgkin's disease and a minority of neoplasms previously referred to as "histiocytic" lymphomas. The majority of these "histiocytic" lymphomas are now known to be lymphoid malignancies involving the B-cell series.

The initial impetus to create a work combining aspects of cel lular immunology with their clinical applications grew from the ed itors' discussions of the area's needs with many of the leaders in the field over a period of time. From the nucleus of ideas that emerged, we have here attempted to create a unified and inte grated coverage of the rapidly growing field of cellular immunology research and to trace out-from what seems at times a genuine plethora of important new findings-the many and often impor tant clinical implications. Because of this approach, the chapters of Clinical Cellular Im munology attempt to be more than critical reviews of research and clinical data, going beyond analysis to synthesize working hypotheses about the functional meaning of cellular immunological phenomena and their likely clinical significance. To accomplish this undertaking, the text begins first with a consid eration of the molecular aspects of antigen recognition (Luderer and Harvey) and of the ensuing regulatory program initiation (Fathman). Then, the functional subsets oflymphocytes as they in teract to produce and control the developing immune response are explored in detail (Sigel et a1.), followed by a unique analytical dis section of the action of immunosuppressive agents on the sundry inductive and regulatory immunologic pathways (Sigel et al.). A majority of the data and conclusions drawn by the authors in the previous chapters arise from work on murine systems, al though wherever appropriate, human data has been introduced.

When we were first approached by the senior editors of this series to edit a book on interactions between the host and infectious agents, we acceptedthis offer as an exciting challenge. The only condition, readily agreed upon, was that such a book should focus on the immunology of infections in humans. Our reasons, if not biases, were severalfold. We sensed that the fields of microbiology and im munolgy, which had diverged as each was focusing on its individual search, were coming together. In agreement with the opinions expressed by Dr. Richard Krause in the Introduction, we strongly believed that the development of the immune system evolved in response to infectious agents and that the evolution of these agents was influenced in turn by the character of the host's responses. An inten sive examination of the multitude of primitive or more recently developed host defense mechanisms to determine their relative contribution to man's resistance to a given infectious agent appeared to us to be of crucial basic· and practical interest. Many immune mechanisms studied in animals were being explored in humans and it appeared timely to focus particularly on what was known about man's resistance to infectious agents, correlating this information with lessons learned from relevant experiments in animal models.

In 1966, the first Germinal Centre Conference was held in Bern, Switzerland, as a result of an initiative from C. C. Longdon of Oak Ridge, Tennessee and H. Cottier of Bern. Fifteen years later, in June of 1981, the 7th conference in this series was held in Groningen, the Netherlands. At this "International Conference on Lymphatic Tissues and Germinal Centers in Immune Reactions," instead of focusing on germinal centres only, the intricate relationships between various constituents of the lympho-myeloid complex consti tuted the theme of the meeting. As Yoffee stated in 1966, "Perhaps we should consider not only the germinal centre itself but the surrounding tissue as well. " Over the years, these conferences have brought together a small but dedicated group of scientists, sharing (1) a fundamental interest in the physiology of the lymphoid system proper, and (2) the recognition that, only in the in vivo situation can the final answers be obtained, however ingeniously in vitro experiments may mimic reality. In 1966, the conference lasted three days and included 57 contributions "which were discussed at length. " The present conference accomodated 62 free papers and 64 poster presentations as well as some 24 additional abstracts selected from more than 200 submissions. Prominent among the selection criteria was the adage "in vivo veritas. " This volume represents the presentations from the 1981 Germinal Centre Conference.

This volume is arecord of the proceedings of a NATO Advanced Study institute on "Biochemical and Bio logical Markers of Neoplastic Transformation" held September 28 - October 8, 1981, at Corfu, Greece. As early as 1860, Rudolf Virchow provided the first genetic concept of cancer by postulating "Omnia ceZZuZa e ceZZuZa ejusdem generis", a modification of the then exisiting cell theory "Omnis ceZZuZa e ceZZuZa". Thus, the idea that all cells originate from the parent cell was extended to the idea that all cancer cells come from the "paren t" cancer cello But how the first cancer cell arose, or in other words, how anormal cell changed to a cancer cell, is, even after 120 years, a mystery. Experimental studies of the past have convinced us that a number of factors contribute to the neoplastic transformation of anormal cell, but our knowledge on the mechanisms involved in this process is still in an embryonic state. In the last few years, however, this field has witnessed a most remarkable advancement cata lyzed by the development of modern technology in the al lied fields of immunology, the production of mono clonal antibodies, molecular biology, and sequencing v PREFACE and cloning of ONC genes. Presently, it is becoming more and more evident to the wishful mind of those engaged in this research that we are approaching a turning point. Thus, an assessment of the present situa tion will be most desirable at this time
