Proceedings of a symposium on management lessons from engineering failures, presented at the ASCE National Convention, held in Boston, Massachusetts, October 28, 1986. Sponsored by the Engineering Management Division of ASCE. This collection contains six papers describing failures associated with the technical quality or functional capability of the project as designed and constructed. Engineering failures are not always catastrophic. More of them are attributable to improper management of contracts than to actual physical failures. While these failures involve technical issues, they also involve management issues, and communication problems are often a contributing cause. These papers examine failures to show what caused the failures, what resulted from them, and, in some instances, what could have been done to prevent them.

Engineering Disasters – Lessons to be Learned shows that there is always something to be learned from disasters. In this practical and highly relevant text Don Lawson has provided Thoroughly researched accounts of well-known disasters and failures worldwide Valuable interpretative sections, drawing out the lessons to be learned in each case Examples from a wide range of industries Background information and views of other experts in the field An excellent source of references for further study Common threads and conclusions from accident investigations Humans design, build, operate, use, maintain and can wreck engineering products. Humans are fallible. Engineers have to take into account all the potential failures of people, including other engineers, as well as failures of equipment and materials. Design engineering is a structured process using both art and science to create new or improved products – building on experience, bad as well as good. Failure occurs when something or someone fails to perform to expectations.

Saving Human Lives written by Robert E. Allinson and has been published by Springer Science & Business Media this book supported file pdf, txt, epub, kindle and other format this book has been release on 2006-05-20 with Philosophy categories.

This is a pioneering work. Recent disasters such as the tsunami disaster continue to demonstrate Professor Allinson’s thesis that valuing human lives is the core of ethical management. His unique comparison of the ideas of the power of Fate and High Technology, his penetrating analysis of the very concept of an "accident", demonstrate how concepts rule our lives. His wide-ranging investigation of court cases and government documents from the seventeenth through the twentieth centuries, and from places as diverse as the USA, UK and New Zealand provide ample supporting evidence for the universality and the power of explanation of his thesis. Saving Human Lives will have an impact beyond measurement on the field of management ethics.

The System Safety Skeptic written by Terry L. Hardy and has been published by AuthorHouse this book supported file pdf, txt, epub, kindle and other format this book has been release on 2010 with Industrial safety categories.

Advanced technologies and increasing automation have forever changed how systems work and how people interact with them. Transportation systems, energy extraction and production systems, medical devices, and manufacturing processes are increasingly complex. With the use of these complex systems comes increased potential for harm to humans, property, and the environment. System safety is a widely accepted management and engineering approach to analyze and address risks in these complex systems. When used correctly, system safety methods can provide tremendous benefits, focusing resources to reduce risk and improve safety. But poor system safety analyses can lead to overconfidence, and can result in a misunderstanding of the potential for harm. The System Safety Skeptic describes critical aspects of the discipline of system safety, including: Safety planning Hazard identification Hazard risk assessment and associated risk decision making Risk reduction and hazard controls Risk reduction verification Hazard tracking and anomaly reporting Safety management and culture Accidents in multiple industries and organizations are used to illustrate potential missteps in the system safety process, including: Failure to plan and implement systematic safety efforts, and failure to plan for emergencies Failure to accurately identify the hazards and what can go wrong Underestimating the chances that an accident could happen Underestimating the worst possible outcomes Overestimating the effectiveness of safeguards Failure to properly verify that safeguards actually work Failure to learn from the past Failure of the organization to adequately manage system safety efforts This book provides hundreds of lessons learned in safety management and engineering, drawing from examples from many industries as well as the author's years of experience in the field. These real-world lessons help foster a healthy skepticism toward safety analysis and management in order to prevent future accidents.

Learning From Failures written by Ashraf Labib and has been published by Elsevier this book supported file pdf, txt, epub, kindle and other format this book has been release on 2014-06-19 with Technology & Engineering categories.

Learning from Failures provides techniques to explore the root causes of specific disasters and how we can learn from them. It focuses on a number of well-known case studies, including: the sinking of the Titanic; the BP Texas City incident; the Chernobyl disaster; the NASA Space Shuttle Columbia accident; the Bhopal disaster; and the Concorde accident. This title is an ideal teaching aid, informed by the author’s extensive teaching and practical experience and including a list of learning outcomes at the beginning of each chapter, detailed derivation, and many solved examples for modeling and decision analysis. This book discusses the value in applying different models as mental maps to analyze disasters. The analysis of these case studies helps to demonstrate how subjectivity that relies on opinions of experts can be turned into modeling approaches that can ensure repeatability and consistency of results. The book explains how the lessons learned by studying these individual cases can be applied to a wide range of industries. This work is an ideal resource for undergraduate and postgraduate students, and will also be useful for industry professionals who wish to avoid repeating mistakes that resulted in devastating consequences. Explores the root cause of disasters and various preventative measures Links theory with practice in regard to risk, safety, and reliability analyses Uses analytical techniques originating from reliability analysis of equipment failures, multiple criteria decision making, and artificial intelligence domains

The overwhelming majority of a software system’s lifespan is spent in use, not in design or implementation. So, why does conventional wisdom insist that software engineers focus primarily on the design and development of large-scale computing systems? In this collection of essays and articles, key members of Google’s Site Reliability Team explain how and why their commitment to the entire lifecycle has enabled the company to successfully build, deploy, monitor, and maintain some of the largest software systems in the world. You’ll learn the principles and practices that enable Google engineers to make systems more scalable, reliable, and efficient—lessons directly applicable to your organization. This book is divided into four sections: Introduction—Learn what site reliability engineering is and why it differs from conventional IT industry practices Principles—Examine the patterns, behaviors, and areas of concern that influence the work of a site reliability engineer (SRE) Practices—Understand the theory and practice of an SRE’s day-to-day work: building and operating large distributed computing systems Management—Explore Google's best practices for training, communication, and meetings that your organization can use
