In pregnancy, maternal nutrition sustains and nourishes the developing child. Imbalances in either the direction of nutritional excess or deficiency can have adverse consequences for child health. In addition, more research now suggests that good pregnancy nutrition influences child health beyond pregnancy and delivery. This includes modifying the risk of child health outcomes as they enter childhood and adulthood through influences on placental development, hormonal pathways, and organ structure and function. Poor pregnancy nutrition may also compromise maternal health during pregnancy, which may have long-term consequences for women’s health. Understanding the biological and social mechanisms operating during pregnancy can help in the design of better clinical and public health interventions. This Special Issue on “The Role of Pregnancy Nutrition in Maternal and Offspring Health” includes etiological and mechanistic studies of pregnancy nutrition with short- and long-term maternal and child health outcomes, including original research, narrative reviews, and systematic reviews and meta-analyses. Together, this body of work provides important insights into the influence of dietary patterns, food groups, and nutrients on pregnancy outcomes, and long-term neurodevelopmental, respiratory, and metabolic health in the children. How to prevent and manage low birth weight Growth and nutrition during the fetal period and the first 24 months after birth are important determinants of development in early childhood. Optimal nutrition and health care of both the mother and infant during these first 1000 days of an infant's life are closely linked to growth, learning potential and neurodevelopment, in turn affecting long-term outcomes. Children with low birth weight do not only include premature babies, but also those with intrauterine growth restrictions who consequently have a very high risk of developing metabolic syndrome in the future. Epidemiology, epigenetic programming, the correct nutrition strategy and monitoring of outcomes are thus looked at carefully in this book. More specifically, two important nutritional issues are dealt with in depth: The first being the prevention of low birth weight, starting with the health of adolescent girls, through the pre-pregnancy and pregnancy stages and ending with lactation. The second point of focus concerns the nutritional follow-up and feeding opportunities in relation to dietary requirements of children with low birth weight.

To date, limited evidence suggests that maternal preconception and pregnancy dietary patterns are associated with reduced risks of adverse maternal and child health outcomes such as gestational diabetes mellitus and preterm birth. However, there are insufficient studies examining preconception and pregnancy dietary patterns on birth weight outcomes such as gestational-age birth weight for any conclusion to be made. Additionally, there is a relative paucity of studies investigating preconception, postpartum, and beyond postpartum dietary patterns. Recent research has advanced from examining each modifiable lifestyle factor singly (e.g. diet, physical activity, smoking) towards a holistic approach of examining multiple lifestyle factors using composite healthy lifestyle scores. Two large-scale mother-offspring cohorts in Ireland and the United States have shown that adherence to a healthy lifestyle during pregnancy and beyond pregnancy is associated with reduced risks of adverse offspring birth outcomes and childhood obesity. These findings are consistent with data obtained from cross-sectional studies based in China and Tanzania.

Obesity is a health issue that is affecting an ever-increasing number of people around the world. Increasing obesity rates are particularly troubling because much evidence has been reported that maternal diet before and during pregnancy can increase the likelihood of obesity in the next generation, through epigenetic and obesity-related developmental changes. Extremely low fat maternal diets have also been shown to have transgenerational effects on health, including evidence for a "thrifty phenotype" hypothesis that supposes the intrauterine conditions prepare the offspring for the outside world based on the nutritional environment available during fetal development; therefore, offspring exposed to low fat intrauterine environments but much higher fat diets outside of this environment are more at risk for obesity and other metabolic dysfunction. In order to test the transgenerational relationship between maternal diet and offspring health, Danio rerio females were fed either a high fat diet (HFD), control diet, or low fat diet (LFD) for several months and crossed with males on the control diet. This study reports that significant differences in offspring early motor behaviour were observed were observed in offspring sired from HFD and LFD mothers, both from the control and from each other. These results indicate possible effects on skeletal muscle and neural development. Significant differences in appetite were also observed between control and HFD, and between HFD and LFD. These results may indicate changes in the secretion of satiety-related hormones in these offspring. HFD and LFD females also showed evidence of decreased fertility and increased offspring mortality rates. These data indicate that maternal diet before and during pregnancy may have far reaching effects on the development and general health of the offspring.

Abstract: The nutritional status of the pregnant women is being recognized as one of the most important factors in the development of a baby, both pre and postnatally. Malnutrition can cause sterility, spontaneous aborption, stillbirth, premature birth, deformities and mental retardation. Statistics from many sources, such as European clinics during WWII and current figures from hospital observations all over the country, have been collected and compiled to provide information on prenatal and early childhood development in relation to nutrition in three ways--physiologically, neurologically, and behaviorally. Low birth weight is the measurement most closely associated with the incidence of neonatal disorders, and is shown to be closely related also to maternal nutrition. Reproductive casualties, particularly in regard to handicapped children, can be greatly decreased by better education and nourishment of expectant mothers.
