On May 14, 2013, the 25th anniversary of our nation's deadliest drunk-driving crash, which killed 24 children and three adults in Carrollton, KY, the National Transportation Safety Board's (NTSB's) five-member board voted unanimously to issue bold recommendations to help the U.S. reach zero and eliminate alcohol-impaired driving. Bold steps are needed: On average, every hour, one person dies in a crash involving a drunk driver and 20 more people are injured, including three with debilitating injuries. That adds up quickly to yearly totals of nearly 10,000 deaths, 27,000 lives forever altered and another 146,000 injured. This safety report and recommendations culminate a year-long effort by the NTSB to thoroughly examine this problem and develop a set of targeted interventions. Figures. Alcohol-impaired driving is an important health and social issue as it remains a major risk to Americans' health today, surpassing deaths per year of certain cancers, HIV/AIDS, and drownings, among others, and contributing to long-term disabilities from head and spinal injuries. Progress has been made over the past decades towards reducing these trends, but that progress has been incremental and has stagnated more recently. Getting to Zero Alcohol-Impaired Driving Fatalities examines which interventions (programs, systems, and policies) are most promising to prevent injuries and death from alcohol-impaired driving, the barriers to action and approaches to overcome them, and which interventions need to be changed or adopted. This report was prepared in accordance with Section 2013 of the Safe, Accountable, Flexible, Efficient Transportation Equity Act: A Legacy for Users (SAFETEA-LU). The report summarizes a series of studies undertaken by the National Highway Traffic Safety Administration to acquire the information needed to address the general problem of drug-impaired driving. The report describes the research conducted on prevention, detection, and prosecution of driving under the influence of drugs; issues associated with determining what drugs impair driving; difficulties in relating blood levels of drugs and impairment; lack of information about what drugs are frequently used by drivers and what drugs elevate crash risk; problems in obtaining representative data about current enforcement, prosecution, and adjudication of drug-impaired driving; training for law enforcement officers in recognizing drug-impaired drivers; review of drug-impaired driving laws; and what is known about the role of drugs as causal factors in traffic crashes. It highlights the need for further research and concludes with recommendations to better address the problem of drug-impaired driving.
