"Diseases associated with impairment of the coronary circulation are very common. Hence, the treatment of such diseases is a matter of considerable importance. It is now considered one of the main problems of clinical medicine. The basis for solving the problem is to enlarge our knowledge of the mechanisms of action of the drugs used to treat coronary insufficiency and to find new effective agents. Owing to the complexity of physiological regulation of the coronary circulation, there are many difficulties of a procedural order in studying it experimentally. This was one of the reasons why ideas on the action of many pharmacological agents were derived until recently from the results of experiments involving the isolated heart. When applied to the intact organism, these ideas proved to be inexact and, at times, wrong. Some ideas on the effects of certain drugs were based on experiments involving the intact organism, but the data proved to be inexact due to the inadequacy of the method selected by the authors to determine the mechanism of action of a given substance. Another reason for misconceptions of the effects of some drugs stemmed from insufficient knowledge and contradictory opinions expressed by various investigators on many aspects of the coronary circulation. Now, however, advances in experimental techniques have produced new facts bearing on the physiological regulation of the cardiac blood supply. This has given rise to the need to reexamine some matters concerned with the influence of pharmacological agents on the coronary circulation, relying on modern ideas concerning its physiological regulation and using adequate experimental techniques. Most drugs have complex mechanisms of action owing to the intimate relationship between the processes regulating the cardiac blood supply. Elucidation of these mechanisms requires the use of various techniques, each of which must be suited to the particular aspect under study. This approach will result in a reappraisal of the action of several pharmacological agents on the coronary circulation. Moreover, we believe that the data in this book will be helpful in making more efficient use of these agents in the treatment of various forms of coronary insufficiency."--Introduction.

by JULIEN IE HOFFMAN One of the earliest coronary physiologists was Scaramucci who, in 1695, postu lated that during systole the contracting myocardium inhibited coronary blood flow. Since then, the many contributions that have been made to our knowledge of the coronary circulation can be arbitrarily divided into three phases based on advances in technical methods. The early phase of research into the coronary circulation, done with great difficulty with crude methods, may be regarded as ending in the 1940s, and it included major discoveries made by such well known investigators as Georg von Anrep, Ernest Starling, Carl Wiggers, and Louis Katz, who formulated much of our basic understanding of the field. After 1940, the field of coronary physiology entered a new phase when instruments for high fidelity registration of coronary flow and pressure became available. This era was domi nated by Donald Gregg who combined careful attention to the function of these instruments (some of which he helped to develop) with an extraordinary ability to discern mechanisms from apparently minor changes in coronary flow and pressure patterns. His book 'The Coronary Circulation in Health and Disease' set a new standard in the field. After 1960, techniques for measuring regional myocardial blood flow became available, and enabled a large group of eminent investigators to make major advances in understanding the physiology and pathophysiology of myocardial blood flow.

Cardiovascular dynamics is a field in which modelling and systems analysis have formed an extremely important discipline. For example, understanding of even such a fundamental function of the circulation as the relationship between central venous pressure apd cardiac output has required evolution of a pertinent model based on years of exhaustive ex perimental investigations by Starling, Starr, and Guyton. Hemodynamic analyses of pulsatile pressures and flows in the arteries and veins have been a continuing challenge taken up by champions of fluid dynamics such as Frank, Wetterer, Taylor, and Wormersley, just to mention a few names, and some kind of model was always proposed as a conceptual framework. An even greater challenge to cardiovascular dynamicists was how to analyze the intermittent coupling of the ventricle and the arterial or venous vasculature through the valve. The availability of numerical solutions by computer and the recently evolved ventricular model with a time-varying elastance and a pressure-dependent internal resistance opened the way to analysis of this coupling. The ever increasing speed of computers has also facilitated trips between the fre quency and the time domain, even on-line for some experimental studies. This book contains many analyses dedicated to the interactions between the heart and the vasculature, providing the reader with findings at the cutting edge of current research in this field.
