This WHO oral health briefing note series focuses on prevention and treatment of dental caries (tooth decay) with mercury-free products and minimally invasive interventions. Dental caries is a major public health problem globally as the disease affects all age groups. The first section of the briefing note series explains why mercury-free materials and products and minimally invasive interventions are important. The following sections of the document describe six essential strategies using mercury-free products and minimally invasive intervention approaches to prevent and treat dental caries: fluoride toothpaste, fluoride varnish, glass ionomer cement sealants, glass ionomer cement restorations, silver diamine fluoride and composite resin restorations. The document was developed in the context of the COP4 of the Minamata Convention on Mercury; a global treaty that aims to protect human health and the environment from emissions and releases of mercury and mercury compounds. The Minamata Convention recommends the use of cost-effective and clinically effective mercury-free products for dental restoration. The briefing note series is not primarily intended for oral health care providers but for non-specialist readers and the public at large.

Restorative dentistry has experienced a shift from the mainly reparative dentistry of the 20th century towards a minimal intervention approach encompassing up to date caries diagnosis and risk assessment before arriving at treatment decision. A shift in philosophy from the traditional surgical model of excision to a more modern medical model of treating caries is occurring so, when operative care is indicated, it should be aimed at "prevention of extension" rather than "extension for prevention". It’s the localized destruction of susceptible dental hard tissue by acidic by-products from bacterial fermentation of dietary carbohydrates. Dental caries is the main reason for placement and replacement of restorations. The demineralization process occurs because of high consumption of sugary and/or starchy foods and sugared beverages, combined with insufficient fluoride exposure, inappropriate oral hygiene, and/or poor salivary flow. The replacement of restorations can result in cavity preparations larger than the predecessors, which leads to weakening of the remaining tooth structure. The longevity of a restoration is attributed to the restorative material properties, the technical quality of the restorative procedure and the patient’s compliance with appropriate maintenance of their oral hygiene. The minimal intervention paradigm emphasizes use of adhesive restorative materials in order to minimize the size of cavity preparation. This new medical model, known as Minimally Invasive Dentistry (MID), is a conservative philosophy that reduces restorative procedure time, pain and stress, and results in decreased patient anxiety. MID emphasizes that initial caries lesions in the enamel can be repaired by increasing the amounts of calcium, phosphate and fluoride in the saliva. Also, when a lesion needs to be restored, MID emphasizes adhesive restorative techniques, which allow removal of the decay in a way that involves minimal loss of healthy tooth structure. The term “minimal intervention” was endorsed by the Federation Dentaire Internationale in a 2002 policy statement and is globally recognized.
