This study investigates human error associated with 599 Naval Aviation maintenance-related mishaps (MRMs) in Fiscal Years 90-99. The Human Factors Analysis and Classification System Maintenance Extension (HFACS-ME) taxonomy was utilized to classify contributory human errors within a robust theoretical framework. Variable Poisson process models are developed to predict MRMs and relationships between the error dimensions are investigated. The results of this study show that the HFACS-ME taxonomy provides an adequate framework for the classification of MRM causal factors; that variable Poisson process models are suitable for predicting future mishaps, and that there are significant relationships between selected causal dimensions; sufficient to warrant further investigation. These results provide information regarding the predicted impact of MRMs on future operational readiness and mission capability. Through being aware of these aspects, decision-makers are armed with the knowledge to make better decisions concerning the preservation and allocation of the resources at their disposal.

Naval Aviation is in the midst of a major transformation as it attempts to come to terms with the demands of maintaining operational readiness in the face of diminishing budgets and reduced manning. Diminishing operating and procurement budgets mean that Naval Aviation is for the most part "making do" with existing aircraft. Over the past decade, one in four Naval Aviation mishaps were partially attributable to maintenance error. The present operating environment underscores the need to address maintenance error and its causes. The current study accomplishes three things. First, it evaluates 470 Naval Aviation mishaps with distinct maintenance error correlates. Second, it categorizes those errors using a taxonomy based upon current organizational and psychological theories of human error. Third, it mathematically models the consequences of these errors and uses the models to (a) predict the frequency with which maintenance-based mishaps will occur in the future and (b) approximate the potential cost savings from the reduction of each error type.

The purpose of this thesis is to examine the development of a Web application to display, analyze, and produce reports of human error involvement and patterns in aviation maintenance mishaps. The Human Factors Analysis and Classification System-Maintenance Extension (HFACS-ME) taxonomy, a framework for classifying and analyzing the presence of maintenance errors that lead to mishaps, is the foundation of this tool. The target audience for this tool includes safety and maintenance personnel, mishap investigators, and safety analysts. A review of five areas is needed to produce the Web-based prototype: (1) client/server architectures, (2) database management systems, (3) Web application design, (4) application coding, and (5) usability considerations for a Web/database tool. Collectively, these topics provided a foundation to develop an effective and user-friendly prototype, referred to as HFACS-ME Web. A usability study was conducted using potential end-users. The participants were given both written procedures to navigate through the prototype and an exit survey. The results of the survey, including objective and subjective responses, indicate strong user support for the HFACS-ME Web prototype in concept and implementation and suggest that the training and analysis capability it provides may contribute to a reduction in maintenance errors and ultimately a decreased mishap rate.

A usability study was conducted using potential end- users. Fifteen Naval Aviation Safety Officers and Naval Aviators were given written procedures to navigate through the prototype and an exit survey. The survey responses, including objective and subjective responses about the prototype were gathered. The results indicate, that with proper training, the prototype could provide insight into maintenance errors, which could be used to target hazards and develop intervention strategies to prevent future mishaps.

The increasing mechanical reliability of Naval Aviation (NA) aircraft has made human error (HE) the leading cause of Class A and B flight mishaps. To increase understanding of the underlying causes of HE, the Naval Safety Center in Norfolk, VA, developed the Human Factors Analysis and Classification System (HFACS). The HFACS taxonomy consists of 17 types of basic HE. This HFACS taxonomy has been used as a data analysis tool to classify 141 Class A and B Naval Tacitcal Aircraft (TACAIR) flight mishaps (FM) from fiscal year (FY) 90 and FY97. The study shows an important relationship between Adverse Mental State and 12 of the 17 HE types in the HFACS taxonomy. Significantly, when one of these 12 HE types is cited in and FM, greater than 70 percent of the time, Adverse Mental State is also cited as a co-casual factor in the mishap. Two other HE types have this 70 percent co-causal factor relationship with 3 of the 17 HE types. Adverse Mental State has an important relationship to the majority of the HE types in the HFACS taxonomy, compared with other HE types. For this reason, Adverse Mental State merits further investigation and should be considered in the development of HE prevention programs for Naval Aviators.

The investigation and modelling of aviation accident causation is dominated by linear models. Aviation is, however, a complex system and as such suffers from being artificially manipulated into non-complex models and methods. This book addresses this issue by developing a new approach to investigating aviation accident causation through information networks. These networks centralise communication and the flow of information as key indicators of a system’s health and risk. This holistic approach focuses on the system environment, the activity that takes place within it, the strategies used to conduct this activity, the way in which the constituent parts of the system (both human and non-human) interact and the behaviour required. Each stage of this book identifies and expands upon the potential of the information network approach, maintaining firm focus on the overall health of a system. The book’s new model offers many potential developments and some key areas are studied in this research. Through the centralisation of barriers and information nodes the method can be applied to almost any situation. The application of Bayesian mathematics to historical data populations provides scope for studying error migration and barrier manipulation.
