Research in electrophysiologic monitoring in anesthesia and intensive care has focussed mostly on questions pertinent for patient care: First how to quantitate drug effects on brain electrical activity and the degree of anesthetic-induced suppression of the central nervous system. Second, how to monitor functional impairment following cerebral ischemia and hypoxia. And third, how to differ entiate between drug-induced effects on the central nervous system and deleteri ous events related to reductions in cerebral blood flow and/or oxygen delivery. Even though progress has been achieved over the last 10 years in this field and fascinating new techniques have been developed, it is still not clear which monitor parameter will provide adequate information on the depth of anes thesia and the analgesic level. Because the central nervous system has been one of the main research areas in our department over the last 10 years, we organized a workshop to summarize the latest developments in central nervous system monitoring. This book comprises the topics of this workshop and is intended to provide insight into the current status of central nervous system monitoring, elucidating possible indications and delineating its limitations.

In spite of today's increasing body of knowledge in regard to central nervous func tion and/or the mode of action of centrally active compounds, little is done to monitor those patients which are at risk of cerebral lesions either in the OR or in the ICU. Due to the inconsistency of reports regarding the application and the benefits computerized EEG and/or evoked potential monitoring will bring to the clinician, physicians still are reluctant to get involved with a technique, which they think, will have little or no effect on the outcome of a patients well being. However, due to the development in computer technology, data acquisition and comprehension, it now is possible to monitor such a viable organ as the Central Nervous System (CNS) on a routine base without being a specialist in neurology or electroencephalography. Thus, the book is intended to guide the clinician to use BEG and evoked potential monitoring in a day to day situation, without going too deep into technical details. As an improvement of cerebral care is needed, various representative cases underline the interpretation of EEG power spectra and evoked potential changes in regard to the underlying clinical situation. It is hoped that this book will serve as a guide to anyone who considers cerebral monitoring a necessity in today's patient care. This may be the anesthesiologist, the intensive care therapist, the nurse anesthetist as well as the medical personnel in the lCU setting.

During the last decades, research on spinal cord has attracted a great deal of attention because of problems such as sensory-motor and autonomic dysfunctions associated with traumatic and other injuries. Recording of spinal cord evoked potentials is one of the most promising approaches to understand the spinal cord function. However, the details of the evoked potentials and their significance in various pathological conditions are not yet fully characterized. The symposium from which this book originates represents a sig nificant watershed in the field of intraoperative neural monitoring, since the participants concluded that electrophysiologic monitoring techniques should be considered a "standard of care" for surgical pro cedures that place the central nervous system (CNS) at risk for injury. Specifically, it was agreed that the somatosensory-evoked potential (SEP) is a remarkably reliable and sensitive indicator of several aspects of CNS function, and should be routinely employed as an intraopera tive monitor during many neurosurgical and orthopedic procedures. The significance of this conclusion cannot be overstated, for at the time of this writing, intraoperative monitoring methods based on evoked-potential analyses are still considered experimental and are not in routine use. The reasons for this are not clear, given the accu mulation of literature and expertise on this subject over the past five years. Granted, the cost of electrophysiological monitoring equip ment is high, but only initially. The benefits of injury prevention far outweigh these costs, from both medical and economic viewpoints. It is our sincere hope and goal that the medical community be made aware of the value of intraoperative neural monitoring.

The book discusses the application of evoked potentials recording in comatose patients admitted to Intensive Care Units and in patients undergoing surgical procedures that require direct monitoring of central nervous system function. In particular, it addresses patients undergoing neurosurgical operations, such as posterior fossa surgery, operative treatment of intracranial aneurysms, spinal cord surgery, cerebrovascular surgery, and operations on the descending aorta. Evoked potentials have been widely employed in neurology since the computer-assisted averaging technique became readily accessible to neurologists. More recent applications are discussed in this book. These require a different approach for monitoring the unconscious patient, often in a rapidly changing clinical situation. Applications of the different stimulus modalities are discussed in relation to the clinical situation of individual patients. SEPs and BAEPs find the widest application in the operating theatre and in evaluating comatose patients.

The spinal cord has a characteristic structure and functions that are distinct from those of the brain. Its functions are tremendously important since it modulates the peripheral sensory inputs to the dorsal horn, and it gives rise to the ascending pathways transmitting peripheral afferent inputs to the brain, and conveys the descending pathways from the brain both to the lower motor neurons, the final common pathway, and to dorsal horn sensory neurons. In spite of these vital functions, the spinal cord constitutes only a small percent age of the mass of the human central nervous system and is located far from the skin surface, which has obstructed the recording of its electrical activity. Recently, however, important advances have been made in several recording techniques, including epidural recording or averaging methods, allowing both sensory and motor evoked spinal cord potentials in man to be recorded. This volume is based on the papers presented at the Fourth International Symposium on Spinal Cord Monitoring and Electrodiagnosis. Each of these international symposia has brought together many of the specialists involved in this research, with an important increase in the number of participants since the first symposium was held in Toyko in 1981. At the past symposia several attempts were made to standardize data, techniques, and clinical applications and to integrate the new findings into patient care.
