This book presents the morphology, possible mechanisms of formation, and clinical relevance of tumor deposits (TDs) in different organs and parts of the body. It also describes the diagnostic features that are currently available for TDs and discusses the prognostic impact of TDs. The poor prognostic and predictive values of TDs were found to be as strong as distant metastasis in different series. TDs, best characterized by colorectal cancer, are irregular discrete tumor masses that are discontinuous from the leading edge of the tumor. However, even in colorectal cancers, the place of TDs in tumor staging (tumor, lymph node and distant metastasis categories) remains controversial. This book presents the possible mechanisms of TD formation, including neurotrophic and angiotrophic extravascular migratory metastasis, in comparison with the well-known mechanisms of metastasis. It also shows that TDs were identified in esophageal, gastric, salivary gland, head and neck, and breast carcinomas. The possible mechanisms leading to the frequent association of TDs and peritoneal carcinomatosis are discussed. This is a valuable resource for pathologists, oncologists, and surgeons who are looking for detailed information on the latest scientific insights into the pathology and prognostic implications of TDs.

Developmental cancer products (oncodevelopmental markers, ODM) not only serve as diagnostic and prognostic indicators but also may be used to study the nature of the carcinogenic process and the biology of tumors. For many years oncologists have searched for markers of cancer cells that would permit unequivocal recognition of cancer in contrast to noncancerous tissue. The earliest and still most widely used method of identification of cancer tissue or cells is the structural resemblance of cancer tissue to fetal or immature tissue. Pathologists not only recognize cancer by its morphologic similarity to fetal tissues, but also in many instances can relate the behavior of a given tumor to the degree of tissue differentiation. Thus, poorly differentiated tumors that resemble fetal tissue generally grow more rapidly and metastasize earlier than do well-differentiated tumors that more closely resemble adult tissue. In recent years the commonality of fetal and cancer tissue has been extended to products of tumor cells that, can be analyzed by biochemical, immunological, or physiological techniques. Increas ingly, products of cancer cells similar to fetal products are being identified and studied. These products range from cell-surface markers (fetal or differentiation antigens), placental proteins, hormones, and isoenzymes to a multitude of products, such as carcinoembryonic antigen (CEA), alphafetoprotein (AFP), lymphocyte markers, and nucleic acids, such as tRNA, that are produced in small amounts by v vi PREFACE continually differentiating cells in the adult but in much greater amounts by tumors.

This second volume reports on the reaction patterns of lymph nodes in neoplastic and immunodeficient diseases. Based on the contents of volume 1, it presents a detailed survey of lymph node structures and their cellular components under these conditions. The patterns of nodal reactions to the development and spread of cancer have recently been investigated and discussed by several authors. Here, the immediate interactions between tumor tissue and the regional nodes have been assessed in experimental models and in human material. Using modern morphological methods such as im munohistochemistry on the light and electron microscopic level, new insights have been gained into the stepwise process of lymphogenous metastasis. Macrophages/reticulum cells were found to playa signifi cant role in this process, which is duly emphasized. Based on appro priate animal models, one chapter focuses on various subtypes of these cellular elements and their role in the two separate phases of tumor spread and the development of true metastases. The induction of fibronectin in lymph nodes is effected by tumor cells forming a special part of the extracellular matrix. The multifunctional fibronec tin molecule serves as a mediator between tumor cells and fibroblasts, furthering the formation of tumor stroma. This volume also contains a comprehensive survey of primary im munodeficiency syndromes and their nodal manifestations, reference being made to the most recent immunological knowledge.
