The purpose of this document is to support countries in contextualizing and implementing action area 1 of the roadmap and action plan to strengthen the public health workforce, which includes the emergency workforce. Action area 1 focuses on operationalizing the essential public health functions (EPHFs). Countries can use this document to identify their priorities relating to the EPHFs, subfunctions and public health services in the post-COVID-19 era and to understand and strengthen public health capacities and stewardship. A strategic review of the EPHFs can inform broader public health strengthening or reform. This document also includes an annex summarizing the findings of a survey on mapping EPHF-related health workforce strengthening activities within WHO and by key partners, which informed the work. This document summarizes the technical details and approaches presented in the publication Application of the essential public health functions: an integrated and comprehensive approach to public health, which was co-developed by WHO’s Health System Resilience and Essential Public Health Functions Team, Special Programme on Primary Health Care, and the International Association of National Public Health Institutes.

This roadmap is a statement of a collective vision to develop a shared understanding on the definition, classification and scope of work of the workforce which delivers the EPHFs, and the subsequent collaborative activities to build capacity of this workforce through competency-based education. The roadmap is developed with, and for, countries including key stakeholders such as associations of national public health institutes and schools of public health and proposes a differentiated and progressive approach for countries acknowledging varying capacities and contexts. Across the action areas, this will aid countries to benchmark themselves and assess their current situation; facilitate provision of guidance and tools; and support progress to full implementation towards a strengthened public health workforce delivering all EPHFs for UHC, health security and improved health and wellbeing. TARGET AUDIENCE: Policy-makers, health workers and key stakeholders within and outside the health sector involved in public health and emergency preparedness and response.

Experience with public health emergencies such as the COVID-19 pandemic clearly demonstrates that weak public health capacities leave populations and health, economic, and social systems vulnerable. Health system challenges are increasing in number and complexity, while health system resourcing, often seen as a cost rather than an investment, remains inadequate. The limited resources available are skewed towards clinical services and emergency response, leaving persistent weaknesses in preventive, promotive and protective capacities. World Health Assembly resolution WHA69.1 of 2016 provided the World Health Organization (WHO) with a mandate to support Member States to strengthen the essential public health functions (EPHFs) while recognizing their critical role in achieving universal health coverage. This has been reaffirmed in the Declaration of Astana on Primary Health Care, 2018, and by global partners since, creating an impetus towards and need for guidance in strengthening public health stewardship and capacities informed by the EPHFs. This technical package provides a range of technical resources and flexible tools in relation to EPHFs, to support comprehensive operationalization of public health in countries. The unified list of essential public health functions (EPHFs) consists of 12 activities that can be used to operationalize public health in a country. This comprehensive approach to public health orients health systems to population need and health system risks, and governments and societies towards health and well-being. This maximizes health gains within available resources and builds resilience, while reducing population vulnerability and the overall burden on the health system. The EPHFs can be used to plan public health systems, strengthen stewardship and coordination for public health delivery at national and subnational levels, and integrate public health capacities within health and allied sectors. The EPHFs anchor protective, promotive and preventive capacities within health systems while leveraging multisectoral efforts for health. In this way, strengthening health systems with the EPHFs is central to the primary health care approach and supports the achievement of universal health coverage, health security and healthier populations in tandem.

How are public health services in Europe organized and financed? With European health systems facing a plethora of challenges that can be addressed through public health interventions there is renewed interest in strengthening public health services. Yet there are enormous gaps in our knowledge. How many people work in public health? How much money is spent on public health? What does it actually achieve? None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. The questions the volume seeks to answer are: o How are public health services in Europe organized? Are there good practices that can be emulated? What policy options are available? o How much is spent on public health services? Where do resources come from? And what was the impact of the economic crisis? o What do we know about the public health workforce? How can it be strengthened? o How is the quality of public health services being assured? What should quality assurance systems for public health services look like? This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. Disasters such as earthquakes, cyclones, floods, heat waves, nuclear accidents, and large scale pollution incidents take lives and cause exceptionally large health problems. The majority of large-scale disasters affect the most vulnerable populations, which are often comprised of people of extreme ages, in remote living areas, with endemic poverty, and with low literacy. Health-related emergency disaster risk management (Health-EDRM) [1] refers to the systematic analysis and management of health risks surrounding emergencies and disasters; it plays an important role in reducing hazards and vulnerability along with extending preparedness, response, and recovery measures. This concept encompasses risk analyses and interventions, such as accessible early warning systems, timely deployment of relief workers, and the provision of suitable drugs and medical equipment, to decrease the impact of disaster on people before, during, and after disaster events. Disaster risk profiling and interventions can be at the personal/household, community, and system/political levels; they can be targeted at specific health risks including respiratory issues caused by indoor burning, re-emergence of infectious disease due to low vaccination coverage, and gastrointestinal problems resulting from unregulated waste management. Unfortunately, there has been a major gap in the scientific literature regarding Health-EDRM. The aim of this Special Issue of IJERPH was to present papers describing/reporting the latest disaster and health risk analyses, as well as interventions for health-related disaster risk management, in an effort to address this gap and facilitate major global policies and initiatives for disaster risk reduction.

State of the world’s nursing 2025 provides updated data and evidence on the global nursing workforce. The level of data reflects a 33% increase in the number of countries reporting on a core set of indicators as compared with State of the world’s nursing 2020. The improved data allows greater precision in describing the challenges to nursing education, employment, service delivery and leadership, and for outlining the policy responses to address them. The findings in State of the world’s nursing 2025 provide stakeholders with data, analyses and policy options to take forward in their countries and contexts. Mobilizing the advised sustainable long-term domestic and external investments in nursing can support the needed scale up of nursing education and employment to offset shortages, stem nurse under- and unemployment, improve working conditions and help manage international migration. These investments will stimulate economic growth by creating decent jobs for nurses and greater economic participation by women in the workforce. Such advancements can accelerate progress towards health equity, gender equality and the broader Sustainable Development Agenda.

The Global Health Emergency Corps (GHEC) was launched in May 2023 by WHO with Member States and partners as part of the efforts to strengthen the global health emergency preparedness and response architecture. GHEC is anchored within the broader WHO Health Emergency Prevention, Preparedness, Response and Resilience (HEPR) framework and embraces an approach to emergency coordination and collaboration that recognizes the need to collaborate across the core capability areas of collaborative surveillance, community protection, safe and scalable care, and access to countermeasures (the ‘5Cs’). The Global Health Emergency Corps describes the body of experts in ministries and agencies in every country who work on health emergencies and the global ecosystem through which they coordinate. The GHEC framework is designed to provide guiding principles for standardizing health emergency workforce structures to strengthen the capacity of countries in responding to health emergencies, and to enhance collaboration between countries by better connecting regional and global surge response mechanisms, facilitating information exchange, and improving access to expertise and human response capacity at times of need. This is the first version of the GHEC framework and is intended to be updated as experience is gained with its implementation and adaptation.
