The circulatory system provides the tissues of the body with oxygen and nutrients for survival, allows for the dissemination of inflammatory cells, and clears metabolic waste from building up and damaging tissue. Atherosclerosis is a chronic inflammatory disease that affects medium and large-sized arteries. It begins after birth and the progression depends on several factors - traditional triad: hypertension, hyperlipidemia and diabetes mellitus, then age, sex, smoking and sedentary life-style. At the beginning atherosclerosis is asymptomatic and we cannot estimate appropriately its frequency, but its complications - coronary artery diseases, cerebrovascular diseases, peripheral arterial diseases, which occur late, are responsible for more than half of the yearly mortality in the world. Unfortunately, sudden cardiac death may be the first clinical manifestation. The incipient event is endothelial dysfunction, as a result of injury, caused by high level of cholesterol [especially w-density-lipoprotein LDL], hyperglycemia, hypertension, smoking, infectious agents, and toxins. Endothelial cells overexpress adhesion molecules - vascular cell adhesion molecule-1 [VCAM-1] and increases recruitment of inflammatory cells - monocytes [Mo], T-cells and subsequent release of monocyte chemo-attractant protein-1 [MCP-1] that results in additional leucocytes recruitment. Injured endothelium allows migration of inflammatory cells that release cytokines and lipids into the intima. That leads to cytokine-mediated progression of atherosclerosis and oxidation of LDL. Macrophages [MP] take up oxi-LDL and form foam-cell. They have metabolic activity and produce cytokines, proliferation of smooth muscle cells and formulate athero-fibrose plaque. Atherosclerotic plaque is composed of superficial layer - fibrose cap and lipid core, that consists of foam cells, extracellular lipid and necrotic cellular debris. It progresses as a result of accumulation of lipid and proliferation of smooth muscle cells and results in luminal narrowing of the arteries which leads to compromised blood and oxygen supply to the tissues. The gradually growing atherosclerotic plaques have thick fibrose cap and are stable. They cause symptoms of stable angina. Rapidly growing plaques cause unstable coronary artery disease. These plaques are mainly composed of lipids and have tiny fibrose cap that is prone to fissuring or rupture. Intraplaque hemorrhage from microvessels in plaque initiate platelet adhesion and activation of coagulation cascade that leads to platelet thrombus formation, i.e. promote thrombogenesis. Knowledge of the pathogenesis of the atherothrombosis modifies the diagnostic and therapeutic approach. Conclusion: Attention should be focused on the management of three points: 1. Endothelial dysfunction [correction of modified risk factors: hypertension, hyperlipidemia, diabetes mellitus, life-style-smoking, physical activity and food], 2. Atherosclerosis [modification of the inflammatory cascade, i.e. elimination of inflammatory pathways and inhibition of oxidation of LDL], 3. Thrombogenesis [inhibition of platelet adhesion, activation and aggregation].

Atherosclerosis is the principal underlying cause of cardiovascular and cerebrovascular disease in people of the Western world. Cigarette smoking has been implicated in both the initiation and exacerbation of the atherosclerotic process. Data to support this implication derives primarily from epidemiologic studies where the relationship between the incidence of atherosclerosis in people who smoke cigarettes has been shown to have a strong correlation. There are few well established explanations for this phenomenon, and basic molecular, biochemical, and cellular mechanisms associated with smoking and the development of atherosclerosis remain both undefined and virtually unexplored. Even the epidemiologic correlation between cigarette smoking and the development of atherosclerosis needs further critical studies. It is known that individuals who do not smoke cigarettes develop athero sclerosis and it is also known that in people who smoke but have normal or low blood cholesterol/lipoprotein levels, the incidence of development of atherosclerosis is no different from that which is found in a non smoking population. Answers which explain such observations must address fundamental biological mechanisms. Toward this end, the purpose of this volume is to assemble, in a single publication, information which will address the questions; what basic cellular and/or molecular mechanisms are associated with the development of atherosclerosis and how does cigarette smoking influence such mechanisms to initiate or exacerbate the atherosclerotic process? This volume contains the papers which were presented at the First Altschul Symposium, Atherosclerosis: Cellular and Molecular Interactions in the Artery Wall. The symposium was held in Saskatoon, at the University of Saskatchewan in May 1990 in memory of Dr. Rudolph Altschul, a pioneer in the field of vascular biology and the prevention of atherosclerosis. Dr. Altschul was Professor and Head of the Department of Anatomy at the University of Saskatchewan from 1955 to 1963. The challenge for biomedical scientists is to unravel the multifactorial etiology of atherosclerosis. For the last hundred and sixty years, anatomical pathologists have carefully studied the morphological changes of the human vascular wall during the initiation and evolution of the fibrofatty atherosclerotic plaque. Based on these elegant morphological observations, theories on atherogenesis were put forth by pathologists in the 1840's. Rudolf Virchow suggested that the movement of substances from the blood into the vessel wall was important for atherogenesis while Carl von Rokitansky felt that the deposition of substances on the lumenal surface of the artery resulted in the formation of atherosclerotic plaque. Since these original theories, it has become apparent that the pathogenesis of atherosclerosis is multifactorial and the disease evolves in stages. It is also likely that not all plaques arise through the same sequence of events and that many steps are involved in the development of each plaque. Today, our understanding of the complicated processes of atherogenesis is still incomplete.

A vast literature has been concerned with arteriosclerosis and yet, many aspects of pathogenesis and of the mechanism of development of the arteriosclerotic vascular lesion remain only poorly understood. In recent years, our knowledge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. It is also because of the development of new drugs that interfere with the atherogenic process and thereby prevent the development of the disease or halt its progression.
