Also included is a section to chart your medications and details of your next of kin in case you need to be admitted to hospital, you can bring the book with you to help the doctors looking after you. There is also a section to list any allergies you may have which is important information new doctors need to know. The scoring system on the charts in this book are charts designed by the Royal College of Physicians. It is a national chart and is used by many hospitals up and down the country, using the scoring system to alert them when the patient needs medical attention. This book is for anyone who wants to record their vital observations such as blood pressure, pulse, temperature, heart rate, oxygen saturations, blood glucose levels and weight. It is designed to be used as a tool for you and your doctor working together. It can also be used as a personal health profile diary to take with you hospital, to show the doctors your previous recordings, list of medications allergies, and next of kin etc. The book has enough charts for six months worth of twice daily observation recordings, or a year's worth of daily recordings.
