Catheter ablation is widely accepted as an effective and safe form of therapy for cardiac arrhythmia. In many instances this curative procedure is considered as the first line of therapy if not the ultimate treatment of choice. With the use of radiofrequency (RF) modality; which has revolutionized the technology from a barotraumatic, potentially injurious procedure using high voltage, direct-current (DC) shock to a safe and relatively painless one; catheter ablation procedure now carries a very low risk and is extremely effective for certain types of arrhythmia. Its efficacy rate in curing supraventricular tachycardia involving an accessory pathway or dual atrioventricular nodal pathways has been near perfect and its application for certain types of atrial and ventricular arrhythmia have also been very satisfactory. However, conventional RF ablation has several well known limitations, most notably is its ability to only produce relatively small, point lesions; rendering it effective only for an arrhythmia with a small and/or a superficial target. It was soon recognized that the technology would not likely to have significant utility in arrhythmia with a more widespread target such as atrial fibrillation or those which involve scarred and deep myocardial tissue such as ventricular tachycardia. Indeed, the application of conventional RF technology in these complex but common arrhythmia has yielded unsatisfactory results.

In 1992, clinical cardiac electrophysiology became a recognized sub-speciality of the American Board of Internal Medicine. The formal recognition of this highly specialized and technical field of medicine represents the culmination of thirty years of remarkable scientific and intellectual discovery. Beginning in the 1950s, cardiologists realized that cardiac arrhythmias were the cause of significant morbidity and the sudden death of at least 350,000 patients every year in the United States alone. At that time the only tools available for analyzing abnormal heart rhythms were the standard EKG machine and careful deductive reasoning. During the early 1960s, cardiac pacemakers reflected the first foray in the electrical therapy of cardiac arrhythmias. Pacemakers were first implanted in order to control syncopal episodes related to bradycardic heart rhythms. Although crude and bulky devices, their utility was immediately obvious to physicians and patients alike. The recognition that electrical signals could be recorded from inside the heart and that the heart's rhythm could be controlled by the application of electrical energy began the era of clinical cardiac electrophysiology which was to follow. In the late 1960s and early 1970s and at the peak of the Vietnam conflict. a group of cardiologists with special training in cardiac electrophysiology were sequestered at the US Public Health Service Hospital at Staten Island.

Improving our insights into the genetic predisposition to cardiovascular disease is one of the most important challenges in our field in the next millennium, not only to unravel the cause of disease but also to improve the selection of patients for particular treatments. Nowadays, for example, subjects with a cholesterol above a particular plasma level are exposed to a cholesterol lowering regime based upon the beneficial outcome of epidemiological studies which include subjects not prone to the disease, despite a plasma cholesterol above the accepted level. Identification of the patients who are genetically predisposed to the consequences of this disorder will reduce the number of subjects unnecessarily treated and, hence, the costs of health care. Because in most cardiovascular diseases the genetic component is a consequence of more than one gene defect, only limited progress has as yet been made in identifying subjects genetically at risk. For example, in hypertension only in less than 10% of the patients the genetic defect has been identified. It has been known for quite some time that in heart and blood vessels fetal genes are as high blood pressure and upregulated or induced when they are exposed to such disorders ischemia. Little is known about the function of these genes in the cardiac and vascular adaptation to these disorders; only guesses can be made.
