Latino infants suffer from lower immunization levels than non-Latino White infants. In 1993 and 1994, Latino infants had the lowest immunization levels in San Diego County (43% and 57% respectively), and according to county DHS statistics, represented 45% of pertussis cases in 1993 (Building Healthier Futures Data Supplement, 1996). This prospective randomized study was designed to assess the effective of an immunization educational intervention delivered to Latinas in their third trimester of pregnancy on subsequent initiation of their infant's immunization series. Timely initiation of the immunization series is associated with receiving subsequent immunizations at the recommended time intervals. The study hypothesis was that providing Latinas with immunization education and promoting timely initiation of their infant's immunization through incorporation of components of the Health Belief Model would result in an increase in the proportion of infants that are up-to-date by the recommended two-month age marker. The rationale for providing immunization information shortly before birth of the child is based on the concept of a teachable moment, a period when individuals actively seek out information and are motivated to learn. The intervention was effective in increasing knowledge of the immunization schedule and supports the concept that the third trimester of pregnancy is a highly teachable moment. However, the lack of significant association between the intervention and initiation of the immunization series raises questions about the relative importance of parent education in timely immunization, particularly in clinics with relatively high immunization levels. Prior to this study, the role of providing pregnant women with immunization education and skills to facilitate timely initiation of their infant's immunization series had not been formally evaluated in a randomized prospective study. This study contributes information about educational messages and behavior to the knowledge base of immunization research. It demonstrates that factors other than parental knowledge of immunization timeliness contribute more to immunization up-to-date status than knowledge of immunization timing itself. Focusing on WIC-immunization linkages and on clinic and health care provider practices in determining successful immunization appear to be much more powerful and effective in terms of future interventions.
