Évaluer l'influence de la membrane épirétinienne sur l'efficacité des anti-VEGF dans le traitement de la DMLA exsudative. Nous avons rétrospectivement inclus dans l'étude des patients naïfs de tout traitement. Les yeux ont été divisé en 2 groupes selon la présence ou l'absence d'une membrane épirétinienne. L'acuité visuelle (AV), l'épaisseur maculaire centrale (EMC) ainsi que le nombre d'injections ont été relevés lors du diagnostic, à 12 et 24 mois. Des analyses en sous-groupes selon la molécule anti-VEGF reçu ont été réalisées. Un total de 202 yeux de 173 patients a été inclus dans l'étude parmi lesquels 42 yeux porteurs d'une MER comparés à 160 yeux dépourvus de MER. Le gain d'acuité visuelle après 24 mois était significativement plus important chez les patients dépourvus de MER (p=0,01). Parallèlement, le nombre d'injections intravitréennes était également plus élevé chez ces patients (p=0,01). Les analyses en sous-groupes ont révélé, au sein des patients traités par aflibercept, une différence significative en termes de gain d'acuité visuelle (p=0.03), nombre d'injection cumulées (p=0.005) et EMC moyenne à 24 mois (p=0.01); contrairement aux patients sous ranibizumab parmi lesquels la différence entre les 2 groupes fut moins importante et par conséquent non significative. La présence d'une membrane épirétinienne parmi les patients atteints d'une DMLA exsudative pourrait freiner la récupération visuelle et accroitre le nombre d'injections intravitréennes. Cette observation semble être atténuée chez les patients traités par ranibizumab.

Age-related macular degeneration (AMD) is one of the most common causes of severe visual loss in middle and old-age population, and often leads to serious deterioration in quality of life. Currently, the first-line treatment for neovascular AMD (nAMD) are intravitreal injections of anti-vascular endothelial growth factor (VEGF) medications, including bevacizumab, ranibizumab, and aflibercept and also latest commercially available drug, brolucizumab. During initial examination and imaging and treatment follow-up for patients with nAMD, optical coherence tomography (OCT) is used to predict and assess the therapeutic response and guide the treatment. Several OCT-based biomarkers, including the central subfoveal thickness (CSFT), the presence of intraretinal cysts (IRCs) or subretinal fluid (SRF), and the presence of pigment epithelial detachment (PED), were found to influence baseline visual acuity or visual improvements. Recent analyses of large randomized control trials (RCTs) summarized the usefulness of these OCT-based biomarkers. However, many of these early studies relied on time-domain OCT to evaluate the retinal structures thus providing less precise evaluation of the retinal details. After introduction of spectral-domain OCT (SD-OCT) which provided high resolution images, recent studies offered new insights in specific morphological changes and their different impact on visual function in nAMD. For example, these advancement in resolution offered new classification of IRCs into degenerative and exudative which impacts treatment strategy and final outcome in the treatment of nAMD. Moreover, the recent data disclose a substantial difference between RCTs and real-world studies regarding the response to anti-VEGF therapy. In conclusions, IRCs and PED are associated with poor visual improvement in nAMD in a realworld setting. Both IRCs and SRF responded better than PED to anti-VEGF therapy. These observations mandate large longitudinal studies focusing on the usefulness of these high resolution SD-OCT biomarkers in real-world situations.

Retinal conditions, such as age related macular degeneration (AMD), diabetic macular edema (DME), retinal vein occlusion (RVO), and choroidal neovascularization due to pathologic myopia (CNV due to PM) are an important public health concern that threatens the vision of millions of patients in Canada. The mechanism of these conditions involve the new formation of blood vessels in the retina that eventually leads to loss of vision. Anti-vascular endothelial growth factor (Anti-VEGF) inhibits this growth and allows the restoration of vision. Currently in Canada, two licensed anti-VEGF agents are available in the market, ranibizumab and aflibercept. Bevacizumab, on the other hand, has been developed as an anti-cancer drug. However, its close molecular resemblance to ranibizumab and identical mechanism of action has made it a widely used option, especially in environments that are strained on health resources. However, despite the wide use of bevacizumab for retinal conditions, and the availability of several high-quality randomized controlled trials for its efficacy, Bevacizumab still lacks a Health Canada review for retinal indications. The bevacizumab product monograph carries a warning regarding the intravitreal use of bevacizumab, citing increased risk of ophthalmic complications. In addition, the intravenous use of bevacizumab in cancer patients is often associated with increased risk of thromboembolic events (e.g. stroke). The CADTH therapeutic review titled "Anti-Vascular Endothelial Growth Factor Drugs for the Treatment of Retinal Conditions" established that the efficacy of bevacizumab is not different than ranibizumab or aflibercept, and did not observe any signals indicating issues regarding bevacizumab comparative safety. However, the CADTH recommendation report for the therapeutic review identifies the lack of large randomized trials powered to detect differences in harms outcomes as a research gap. The statistical power required to detect a difference in harms outcome can make a randomized clinical trial prohibitive. A review of available evidence regarding safety of bevacizumab from real-world evidence is of high clinical value, as it represents a useful tool for identifying any potential issues regarding the safety of bevacizumab for use in treating retinal conditions. A summary and critical appraisal of studies regarding bevacizumab safety, contrasted with those of ranibizumab and aflibercept, for the treatment of retinal conditions would allow for a more informed and evidence-based policy and clinical decision process.
