This is the second in a series of three books on advances in prostaglandin research. In recent years there has been an unparalleled interest in these compounds and as a result a vast amount of research data has accumulated since the publication of my earlier book in 1972. At that time it was possible to present a fairly comprehensive review of the various aspects of prosta glandin research in one volume. This is no longer possible and the contents are divided into three volumes; the first one dealing with prostaglandins and reproduction was published in October 1975; the present volume dealing with chemical and biochemical aspects of prostaglandin research, to be published in April 1976 and the third book dealing with physiological, pharmacological and pathological aspects of prostaglandin research wiII be published in May 1976. The authorship represents international scientists consisting of physiolo-. gists, pharmacologists, chemists, biochemists, veterinary scientists and obstetrician gynaecologists actively engaged in different areas of prosta glandin research. An attempt has been made to provide a total coverage of advances relating to prostaglandins. For the sake of completeness and continuity, material covered in the 1972 book is either briefly summarised or reference made to that edition. In recent years there have been notable advances in the chemical and biochemical aspects of prostaglandin research and these are discussed by various authorities in the chapters that follow.

The voluminous literature on prostaglandins includes some excellent reviews. For some time however, there has been a need for the collection in one volume of information from individual experts actively engaged in different aspects of prostaglandin research. This book is an attempt to fulfil that need. In view of the expanding interest and ever-growing accumulation of data of the various effects of prostaglandins it is hoped to revise the book and bring it up-to-date periodically. The largest section is that on reproduction, in which field progress in research and clinical applications thereof have been, and are, of great significance particularly in the area of popula tion control. This is not to decry the skill and efforts of investi gators in other aspects of prostaglandin activity nor the potential biological or therapeutic importance of their findings. In this context, what may appear to be omissions are individual sections on prostaglandin activity in the central nervous system, in the eye and as mediators of the inflammatory response. These are considered in the chapter on General Pharmacology of the Prostaglandins not because they might be considered to be of lesser impact and significance but in order to achieve a rapid publication of the book. S. M. M. KARIM Kampala, Uganda. January 1972. Acknowledgements Tables and Figures previously published are, in general, acknowledged by the reference in the legends and I am grateful to the authors, editors and publishers for their per mission.

There is much evidence to suggest that prostaglandins may have a physiological role to play in the control of thyroid gland metabolism, although the precise nature of this role remains to be defined. Prostaglandins have been found in normal thyroid tissue, and there is evidence of increased levels in some, but not all, cases of medullary carcinoma of the thyroid. which may account for the associated diarrhea. Exogenous prostaglandins, like TSH, bind to thyroid plasma mem branes, stimulate the adenylate cyclase-cyclic AMP system, thyroid hormone secretion, iodide organification, colloid droplet formation, glucose oxidation, and 32p incorporation into phospholipids. Prosta glandins of the E series are the most potent, although their maximal effects are generally less than the maximal effects of TSH. The interrelationships existing between TSH and prostaglandins have been the subject of intensive investigation, but as yet no one uni fying concept has emerged. TSH and prostaglandins have been shown to occupy different receptors on the thyroid plasma membrane. Al though TSH can increase intracellular prostaglandin levels, probably as a result of increased synthesis, prostaglandins do not appear to be essential intermediates in TSH action on adenylate cyclase. The pos sibility that prostaglandins are part of a negative feedback system con trolling TSH action is, as yet, a tentative hypothesis, and further work will be required to unravel the interrelationships between these sub stances that now appear to be far more complex than has been envisaged thus far.

It is clear today that several prostaglandins play an important role in the regulation of many of the physiological events of the reproductive organs in the human. Both naturally occurring prostaglandins and their analogues are used routinely in many countries to ripen the cervix and induce labour at term as well as to dilate the cervix and to terminate pregnancy. Prostaglandin biosynthesis inhibitors are widely used in the treatment of primary dysmenorrhoea. The editors have aimed at an accurate, thorough, yet easily under standable review of the status in 1986 of medical knowledge regarding both the physiological importance and the clinical use of prostaglan dins and their inhibitors in obstetrics and gynaecology. I believe this book will be of value for all clinicians concerned with reproductive health. The list of authors guarantees an authoritative and up-to-date review of this active field. This book was written at the invitation of Dr. H. GOTZE of Springer-Verlag for the series "Monographs in Endocrinology". It is not a comprehensive account of the prostaglandins but has been writ ten with a deliberate emphasis upon those aspects of the field in which I am particularly interested and to which, in some cases, I have made a contribution. I am grateful to Miss E. PFISTERER and her colleagues of Springer Verlag for their excellent work. I should also like to thank my wife without whose patience, encouragement and help this book would never have been completed. Finally this is an appropriate time to express my sincere gratitude to those scientists who over the years have given me samples of prostaglandins-namely Professors S.BERGSTROM and B.SAMUELSSON of the Karolinska Institute, Stockholm, Professor D. A. VAN DORP of the Unilever Research Laboratories, Vlaardingen and Dr. J. E. PIlm of the Upjohn Company, Kalamazoo. Without their help work in this field would have been extremely difficult.

The role of prostaglandins in physiological events and pathological disorders related to human reproduction has been most actively investigated in the past decade. Their clinical use for fertility regulation, extensively evaluated, represents the most common clinical indication for the administration of these remarkable compounds. Thus, it is most appropriate to update the available information related to the use of prostaglandins in the regulation of human fertility to provide a background document for the benefit of clinicians and scientists. Invited experts of international reputation from various parts of the world have contributed, each in his own area of interest, to offer this book, which we hope will fill an existing gap. M. T. M. B. E. S. E. H. ix 1 Potential for prostaglandin use in controlling human reproduction K. T. KIRTON and F. A. KIMBALL BACKGROUND It was observed in 1930 that seminal plasma of a number of species contained very large amounts of substance(s) capable of altering uterine motility. These substances were subsequently demonstrated to be prostaglandins (PGs); this began the early and continued association of this group of compounds with reproduction. The activity was found to be associated with a fraction containing lipid-soluble acids, derived from prostanoic acid. Restrictions imposed by obscure sources and small amounts of material available for testing were initially impediments to their study.

This is the fourth in a series of books on Advances in Prostaglandin Research published by MTP Press. From laboratory and animal studies a large number of practical applications of prostaglandins have been identified. In some areas clinical trials have also been carried out. There is usually a long time interval between identifying practical applications of new drugs and clinical trials. Most of the information on practical applications of prostaglandins is scattered in different journals. Although a number of books dealing with prostaglandin research have been published (including some on practical applications in specific areas, e.g. reproduction) there is no single volume dealing with all areas of practical applications of these substances. This book aims at filling this gap and should be of interest to clinicians as well as to medical and basic scientists in academic institutions and in the pharmaceutical industry. In some areas, exciting potential applications of prostaglandins have been identified from basic research but clinical trials have so far not been carried out. Such areas of practical applications are covered in a separate intro ductory chapter. All contributors to the volume are scientists and clinicians actively engaged in prostaglandin research and most are recognized authorities in specific areas. The need for rapid publication in a fast-expanding field is obvious.
