The introduction of prostaglandins into clinical practice has been one of the most important advances in obstetrics and gynaecology in recent years. During the last decade obstetricians have become familiar with these drugs for inducing labour and for terminating second-trimester pregnancy. Within the last year their use has been extended, in association with antiprogestin, to first-trimester termi nation. Although the effectiveness of prostaglandins in these pharma cological roles is clear, their full potential has still to be explored, and much remains to be learned about their physiology. Prostaglandins playa central role in the initiation of labour. Further clarification of this role could lead to improvements in preventing or treating preterm labour, which still remains the most important cause of perinatal mortality in this country. Prostaglandins are also inti mately involved in the mechanism of menstruation. More detailed understanding of this role should lead to more effective treatments for menorrhagia and dysmenorrhoea. These substances also playa part in the mechanism of implantation, and further research in this area may lead to more effective therapy for infertility.

The role of prostaglandins in physiological events and pathological disorders related to human reproduction has been most actively investigated in the past decade. Their clinical use for fertility regulation, extensively evaluated, represents the most common clinical indication for the administration of these remarkable compounds. Thus, it is most appropriate to update the available information related to the use of prostaglandins in the regulation of human fertility to provide a background document for the benefit of clinicians and scientists. KIMBALL BACKGROUND It was observed in 1930 that seminal plasma of a number of species contained very large amounts of substance(s) capable of altering uterine motility. These substances were subsequently demonstrated to be prostaglandins (PGs); this began the early and continued association of this group of compounds with reproduction. The activity was found to be associated with a fraction containing lipid-soluble acids, derived from prostanoic acid. Restrictions imposed by obscure sources and small amounts of material available for testing were initially impediments to their study.

This is the fourth in a series of books on Advances in Prostaglandin Research published by MTP Press. From laboratory and animal studies a large number of practical applications of prostaglandins have been identified. In some areas clinical trials have also been carried out. There is usually a long time interval between identifying practical applications of new drugs and clinical trials. Most of the information on practical applications of prostaglandins is scattered in different journals. Although a number of books dealing with prostaglandin research have been published (including some on practical applications in specific areas, e.g. reproduction) there is no single volume dealing with all areas of practical applications of these substances. This book aims at filling this gap and should be of interest to clinicians as well as to medical and basic scientists in academic institutions and in the pharmaceutical industry. In some areas, exciting potential applications of prostaglandins have been identified from basic research but clinical trials have so far not been carried out. Such areas of practical applications are covered in a separate intro ductory chapter. All contributors to the volume are scientists and clinicians actively engaged in prostaglandin research and most are recognized authorities in specific areas. The need for rapid publication in a fast-expanding field is obvious.

The voluminous literature on prostaglandins includes some excellent reviews. For some time however, there has been a need for the collection in one volume of information from individual experts actively engaged in different aspects of prostaglandin research. This book is an attempt to fulfil that need. In view of the expanding interest and ever-growing accumulation of data of the various effects of prostaglandins it is hoped to revise the book and bring it up-to-date periodically. The largest section is that on reproduction, in which field progress in research and clinical applications thereof have been, and are, of great significance particularly in the area of popula tion control. This is not to decry the skill and efforts of investi gators in other aspects of prostaglandin activity nor the potential biological or therapeutic importance of their findings. In this context, what may appear to be omissions are individual sections on prostaglandin activity in the central nervous system, in the eye and as mediators of the inflammatory response. These are considered in the chapter on General Pharmacology of the Prostaglandins not because they might be considered to be of lesser impact and significance but in order to achieve a rapid publication of the book. The Eighth Brook Lodge Conference on Problems of Reproductive Physiology is the latest in a series of small meetings held over 18 years. The topics for trlese meetings have reflected the changing interests and backgrounds of the organizers. The recent rapidly increasing quantity of information on the uterine endometrium and its continuing relevance to fertility control stimulated the organizers to focus the 1979 workshop on this topic. The first four speakers provided a histological background for the molecular and biochemical studies described subsequently. The interaction of biochemist, physiologist, pathologist and physician as they focus their thoughts and talents on this limited area of the body is apparent in the dis cussions of presentations. This workshop was planned to provide in sight and a sense of proportion for the participants. For the reader, the record of the proceedings should serve as a useful reference to the state of the art in this area of research. The uterine endometrium is central in the successful implantation and development of the embryo. Examination of the physical relation ships, biochemical interactions and hormonal influences in this tissue could further advance the understanding of this pivotal tissue in the reproductive process. The depth of the understanding of endo metrial physiology should be reflected in our ability to better con trol this reproductive endpoint and to treat its dysfunction. The efforts of many individuals made these Proceedings possible.
