The practice of radiology education: challenges and trends will provide truly helpful gu- ance for those of you involved in teaching and training in radiology. The goal of this book is ultimately to improve patient care. As a companion piece to the first book radiology education: the scholarship of teaching and learning, this book focuses on applying the concepts at a practical level that can be applied flexibly within educational programs for radiology residents and fellows in any medical imaging learning environment. This book focuses on the application of scholarship in terms of the “dissemination of useful, testable and reproducible information to others. ” It links educational theory with practice and for those of you who wish to explore educational practice further, a number of chapters s- gest additional readings and resources. The publication is timely and congruent with one of the most important twenty-first century trends in medical education: the move from amateurism to professionalism in teaching. In the past, medical schools and other health professions’ training institutions have been criticized for their resistance to the adoption of the science of medical edu- tion. Very few of us learned how to teach as medical students and most of us have our teaching responsibilities thrust on us with little preparation. The award of a basic medical degree was assumed to carry with it basic teaching expertise, unfortunately an unw- ranted assumption in some cases.

This is a book about scholarship in the broadest sense. The writing of this book has shown how through scholarship we can bring together academics, practitioners, scientists, radio logists, and administrators from around the world to begin the kinds of conversations that promise to move us to a new way of thinking about and enacting radiology education. Over the past century, we have witnessed tremendous change in biomedical science and the scope of this change has demanded new approaches to medical education. The most significant of the changes in medical education has been a fundamental paradigm shift from a teacher-centered approach to a student-centered approach. This shift, c- bined with the explosion of knowledge, has pressed medical schools to undertake major curricular and institutional reform. The Radiology Department is a pivotal part of any acute and/or comprehensive health care facility. The radiologist can no longer just “hide out” there. Matters of imaging are often public concerns, larger in scope than just the scheduling and managing of a series of image tests. Rather radiology is expensive, often intrusive and in some areas earnestly and endlessly controversial. A radiologist must be attuned to these often confounding contingencies. Two recent developments in the monitoring of education of radiologists can be impacted by the content of this book. For trainees in Radiology, and for that matter, for all trainees in every medical specialty in the U.S., a new accreditation system (NAS) has been put into place under the impetus and aegis of the ACGME, the Accreditation Committee for Graduate Medical Education, the body responsible for graduate medical evaluation and oversight in the U.S. Among its many innovations, the NAS curriculum is concerned with knowledge acquired about social and economic issues pertinent to each specialty. It is also focused on improving communication skills and about enhancing quality and safety. In the elaboration of “milestones” for residency education in these issues are codified into focused initiatives that must be addressed by each trainee as he or she advances in capability and seniority within the training interval.

Those following the profession of radiographer mainly work in the healthcare sector, with image production in medical imaging or with radiotherapy treatments. Radiographers are responsible for patient care and handling technology in this profession al field. Radiographers’ practice is interesting to study in relation to technical developments and changing conditions for performing professional work. The general aim of this thesis was to empirically explore the main features of radiographers’ work, how advances in tech n ology affect radiographers’ practice, interconnections with other practices and students learn in g in practice on the way to becoming professionals. Methods: Data was collected using interviews and observations (Papers I, II & IV). For Paper III, individual interviews were conducted. Data was analysed using a phenomenological interpretative method (Paper I) and practice theory perspective (Papers II–IV). Findings: Radiographers’ professional work with image production was seen as a process comprising three phases: planning the examination, producing the images, and evaluating the images. During this process, radiographers make judgements to ensure patient safety and adapt the technology in use to the individual patient. When conventional imaging techniques are converted into examinations performed by Computer Tomography, the planning phase of radiographers’ work process becomes more important. Technology improvements also mean that the technical aspects of radiographers’ work with image production are easier to foresee in scheduling examinations. The caring aspects however are difficult to plan for because of little information about the patient before the examination. The professional practices involved in medical imaging interconnect to ensure patient safety through materiality and common tasks and/ or projects. The content and quality of two artefacts, the referral and the image, in these interconnections are important in collaborative work to ensure patient safety within medical imaging. Radiography students learn professional knowing in practice i.e. practice-as-work, practice-as language and practice-as-morality, during their clinical placements through alternating between two modes of participation: either observing and listening or acting by themselves. The students developed knowing in practice if the other practitioners allowed them to alternate between these two modes of participation. Implications: The description of radiographers’ general tasks an d responsibilities in a work process can be used for both educational and professionalization purposes. The identified interconnections between involved professions are useful for quality improvement to secure patient safety. The findings about development of knowing in practice can be used in the planning and evaluation of clinical placements for students.
