This book presents important leading-edge research on stomach cancer. Stomach cancer (also called "gastric cancer") is the growth of cancer cells in the lining and wall of the stomach. Stomach cancer represents roughly 2% (25,500) cases of all new cancer cases yearly in the United States, but it is much more common in Japan, Great Britain, South America, and Iceland. It is associated with high salt in the diet, smoking, and low intake of fruits and vegetables. Infection with H. pylori is the main risk factor in about 80% or more of gastric cancers. It is more common in men. A very small percentage of diffuse-type gastric cancers are thought to be genetic. Hereditary Diffuse Gastric Cancer (HDGC) has only recently been identified and research is ongoing, but genetic testing and treatment options are available for families at risk. Metastasis occurs in 80-90% of individuals with stomach cancer, with a five year survival rate of 75% in those diagnosed in early stages and less than 30% of those diagnosed in late stages.

Gastric cancer is one of the most common and potentially lethal gastrointestinal malignancy, but it is now at the forefront of modern oncology due to new discoveries. In recent years, the field of gastric cancer has been transformed by many notable developments in both the diagnostic area and the therapeutic strategies. The main aim of the text is to propose a comprehensive, state-of-the art review of this field, and will serve as a valuable resource for oncologists, pathologists, surgeons and researchers with an interest in gastric cancer. The first chapters will review the interplay between pathogens and especially H. pylori, inflammation, genomic instability, and tumorigenesis. Secondly, an exhaustive overview of the diagnosis of gastric cancer from a clinical/endoscopic and pathologic perspectives will be illustrated. Two specific chapters will deal with tissue and serum biomarkers with an important role in diagnosis as well as prognostic and predictive markers of response to specific therapy. A central section of the book will focus on treatment options available today for patients. Surgery is the cornerstone of treatment for gastric cancer; however, the prognosis of patients with locally advanced disease is still poor even after curative resection. Therefore, in order to improve the prognosis of patients, especially with advanced stage, a multimodal strategy is advisable and is thoroughly addressed in the appropriate section. With the advent of next-generation sequencing and new preclinical model technologies, the genomic landscape of gastric cancer has been delineated and molecular characterizations finalized to novel therapeutic targets of each molecular subtype have been identified. These advances are making it feasible to integrate clinical and phenotype-based diagnostic and therapeutic methods and are described in a dedicated section. The text will conclude with chapters focusing on the future direction of gastric cancer research in the precision medicine era: an in-depth analysis of non-coding RNAs and their role in gastric cancer, the impact of immunomodulation/immunotherapy and the perspectives of nano-medicine will be finally proposed to the audience. A brief review of the existing literature addressing the above mentioned topics will be present in each chapter. This text will serve as a useful resource for physicians and researchers dealing with, and interested in, this challenging malignancy. As someone who has spent nearly half his life wondering about the relationship between Helicobacter and gastric cancer, I find this textbook on the subject exciting and timely. In fact, I am not aware of any other volume that has been able to distil so much new knowledge into such a comprehensive account of a poorly understood field. Taking my own view, as a scientist placed in the middle of the spectrum between basic science and clinical medicine, I can see that the editors, Jim Fox, Andy Giraud, and Timothy Wang, provide a broad mix of expertise, which ensures that the subject is treated with the right balance. From clinicopathologic observations in humans, to epidemiology, through animal models, to molecular and cell biology, this team has hit the mark for most readers. Fox is a well-known leader in animal models with broad expertise. He pioneered the field with observations on Helicobacter species in animals, from the time when only one spiral gastric bac- rium was known, “Campylobacter pyloridis. ” Fox partners with Wang, whose team recently announced a dramatic advance in the field of carcinogenesis—the obser- tion that bone marrow–derived stem cells participate in the changes that become cancer. To this nice mix has been added Andy Giraud from my own country, who brings to the table some remarkable genetic models of gastric cancer based on alterations in the gp130/stat3-signaling pathway.

The problem of gastric cancer, with its multifaceted aspects, offers an example of diversified oncological research, which includes epidemiology, pathogenesis, histo morphology, surgical and conservative therapy in its spectrum.
