Over the past three years, the U.S. Department of Health and Human Services (HHS) has made significant investments in state and local public health in an effort to enhance public health emergency preparedness. The RAND Corporation was contracted to work with the HHS Office of the Assistant Secretary for Public Health Emergency Preparedness (OASPHEP) to develop resources and to prepare analyses to help describe and enhance key aspects of state and local public health emergency preparedness. As part of this contract, RAND was asked to study the response of state and local health departments to recent disease outbreaks -- specifically, Severe Acute Respiratory Syndrome (SARS), monkeypox, West Nile virus, and Hepatitis A -- to address the following questions: (1) How did the public health system in the United States respond to each of these disease outbreaks? What were the roles of federal, state, and local health departments, health care providers, community organizations, and other groups, and how did they interact? (2) In what ways did recent federal investment contribute to public health preparedness? (3) What lessons does the public health response to these outbreaks have for future preparedness, particularly to address the threat of bioterrorism? What improvements are needed to public health infrastructure in the United States and in functional capabilities to address a public health emergency? (4) Was the Centers for Disease Control (CDC) guidance helpful in building capacity for health departments to respond to the outbreaks studied? Are there areas in which guidance is still needed?

West Nile virus is a neurotropic flavivirus that has emerged globally as a primary cause of viral encephalitis. Infection of humans and other vertebrate animals is associated with a febrile illness that can progress to a lethal encephalitis or flaccid paralysis syndrome. Its appearance in the Western Hemisphere in 1999 and the corresponding increase in global disease burden over the last decade have been accompanied by intensive study, including the entry of many scientists into the field. Breakthroughs have been made in understanding the unique transmission pattern between the vector and the multiple avian and mammalian hosts and targets. Studies in mammalian systems have dissected the viral and host factors that determine the pathogenesis and outcome of West Nile virus infection. On the basis of these experiments, progress has been made on the identification of genetic factors that predispose to severe human disease. Thus, in a remarkably short period of time, insight has been gained on a wide variety of disciplines related to West Nile virus biology. The aim of this book was to assemble an up-to-date and cutti- edge anthology from the leading experts in the field. The chapters are balanced by submissions from newcomers who have made significant recent contributions with those from established investigators who have dedicated their careers to the study of West Nile virus.

Before 1999, the West Nile Virus was unknown in the Western hemisphere. Since then, however, each summer has shown it moving progressively across the United States starting in New York. Such recurrent and steady movement means that the virus is here to stay. Transmitted by the bite of infected mosquitoes, the disease causes encephalitis, which is inflammation of the brain. Although most cases are relatively mild and include symptoms like a fever and skin rash, those with more severe cases can exhibit convulsions, muscle weakness, and paralysis. Some cases even prove fatal. There is no specific treatment for the virus, with most therapy coming under hospital oversight. In working to prevent the disease, several areas have taken to spraying against mosquitoes and recommended the use of insect repellent. Nonetheless, the virus continues to show itself and has become a regular, but still dangerous, routine of life.
