Cancer has become the most common disease worldwide. With their extremely aggressive nature of growth, many cancer types portend dismal 5-year survival rates. Chemotherapy, as the conventional treatment option, is the mainstay of treatment for these malignancies, and while some patients derive remarkable clinical response, ultimately chemoresistance prevails. This resistance highlights the self-renewal of tumor cells and reflects its notorious capacity to mediate a microenvironment that is tumor protective. Recently, immunotherapy achieved great successes in tumors with abundant lymphocytes influx, such as melanoma and lung cancer. But, none of the immunotherapeutic regimens have been effective in immune-privileged cancers, reflecting an immune suppressive microenvironment that is lack of immune cell infiltration along with the tumor initiation, development, and progression, as known as ‘cold’ tumor.

This book intends to report new concept of onco-spheres in cancer ecosystem. Onco-spheres are defined as where cancer cells (living organisms) dynamically interact with nontumor cellular (other living organisms) and noncellular components (non-living environmental factors) in the ‘host’ internal environment (habitat) to construct a self-sustainable cancer ecosystem, which can be scoped at three different levels: primary/regional, distal and systemic onco-spheres. Cancer cells should be conceived as ‘living organisms’, interacting with cellular or noncellular components in the host internal environment, not only with the local tumor microenvironment (TME) but also constantly communicating with a distant organ niche as well interacting with the host’s nervous, endocrine and immune systems, to construct a self-sustainable ‘biosphere’, as we termed the tumor ecosystem. By looking at the interaction of cancer and host as a unique ecosystem, we will use ecology principles to further delineate the features of the dynamics of the tumor ecosystem. As the pioneer in proposing this concept, we feel that this full-scale overview of the tumor ecosystem is able to inform the readers about this concept, and to pave the way for designing novel therapeutic strategies on actionable targets within tumor ecosystem. Osteosarcoma (OS) is the most common primary bone tumor, with a bimodal age distribution. The highest incidence is in children and adolescents, and the second highest is among older adults (>60 years). Surgery and neoadjuvant chemotherapy are the main clinical treatments for OS, yet the 5-year survival rate is just 20% for individuals with metastases or recurrent OS. Most standard treatments were established in the 1980s, which can achieve long-term survival in approximately 60% of patients with localized OS. However, in the face of inoperable surgery, we are still limited in what we can do for OS patients who are insensitive or intolerant to chemotherapy drugs. Resistance to chemotherapeutic drugs is also a problem that hinders breakthroughs in OS treatment. Although survival rates have not improved during the past four decades, the heterogeneity and potential molecular aberrations of OS have been revealed as the biology of OS has come to be better understood. With the recent advancements in molecular profiling technology for OS, new technologies and concepts are undergoing sublimation.

Traditionally, surgery, radiotherapy, chemotherapy and hormonal therapy have been used in clinical practice to treat cancer patients. However, these methods are either not completely tumor free or without collateral damage to health tissues. Therefore, tumor recurrence, partial remission, treatment side effect and poor tolerance in elderly patients, and lack of effective strategy to treat late-stage cancers have been the major hurdles faced by clinicians in health care. Although cancer immunotherapy including checkpoint blockade, CAR-T, oncolytic viruses, and recombinant cytokines has taken center stage in mainstream oncology because of its specifically targeting tumor cells without affecting surrounding normal cells, only a proportion of patients receiving treatment respond and others relapse after an initial response. Different tumor indications respond differently, and even in cancer types that respond, unresponsiveness are still observed. This resistance suggests that either lack of sufficient host recognition and immunity (intrinsic) or active immune suppression by tumor complex (acquired).
