Living Standards Measurement Study Working Paper No. 111. This study looks at the socioeconomic background of individual women and draws a correlation between them and the characteristics of their nearest source of family planning. The study assesses the importance of the socioeconomic background and the availability, price, and quality of services on contraceptive use and fertility. In 1969, Ghana was among the first Sub-Saharan African countries to adopt a population policy. Today, the average distance to a source of family planning is still about three miles. Population and fertility growth rates are high, and contraceptive use is low. The results suggest that raising levels of female schooling will also raise contraceptive use and lower female fertility, particularly in rural areas. However, the distances between facilities and related service centers remain a binding constraint on contraceptive use among those in the sample. The study also found no consistent effect on the demand for contraception or on fertility when measuring the quality of services.

Living Standards Measurement Study No. 108. Nigeria, which has experienced high fertility and rapid population growth over the past 30 years, has launched efforts to stem its population growth through contraceptive use. In this study, individual women are linked to the characteristics of the nearest pharmacy, health facility, and source of family planning to assess the importance of the socioeconomic background and the characteristics of nearby services for women's contraceptive use and fertility. The results suggest that limited levels of female schooling are constraining contraceptive use, particularly in rural areas. Policies to raise female schooling will raise contraceptive use and lower fertility. Broader availability of the pill through pharmacy outlets and of the injection and IUD in fixed health facilities is also likely to raise contraceptive use. Consultation fees at health facilities do not appear to be constraining demand for modern methods.

What is the role of family planning interventions on fertility, savings, human capital investment, and development? To examine this, endogenous unwanted fertility is embedded in an otherwise standard quantity-quality overlapping generations model of fertility and growth. The model features costly fertility control and families can (partially) insure against a fertility risk by using costly modern contraceptives. In the event of unexpected pregnancies, households can also opt to abort some pregnancies, at a cost. Given the number of children born, parents decide how much education to provide and how much to save out of their income. We fit the model to Kenyan data, implement several family planning policies and decompose their aggregate effects. Our results suggest that with a small government budget (say, up to 0.5 percent of GDP), family planning interventions might be more cost-effective in improving longrun living standards than policies that subsidise basic education.

Annotation World Bank Living Standards Measurement Study No. 112. Assesses evidence of a negative correlation between the number of children born and levels of child schooling by examining their determinants. In many developing countries, as parents have fewer children, they invest more in the health, education, and welfare of each child. This "quantity-quality tradeoff" is vividly illustrated in the recent economic development of Southeast Asia and Latin America. In Sub-Saharan Africa, however, the existence of such a tradeoff has not been established. The few studies conducted to date reveal either no correlation or a slightly positive one, whereby higher fertility rates are linked to greater schooling per child. This study examines the determinants of fertility and of child schooling in C te d'Ivoire and Ghana to assess evidence of a tradeoff, using data from three surveys conducted between 1985 and 1987. The results are mixed. In C te d'Ivoire, there is evidence of such a tradeoff in urban areas but not rural ones. In urban areas, female schooling, higher income, and improved child survival are associated with lower fertility and higher child schooling. In both rural and urban areas of Ghana, there is a tradeoff between fertility and child schooling with higher incomes, and, in rural Ghana, with increases in mothers' schooling. It also examines the possible trade-offs between cost recovery and the use of health services for different age, sex, ethnic, and income groups. The data, drawn from a multipurpose household survey conducted by the World Bank and Bolivia's statistical office, are used in designing an econometric model to measure the price elasticity of demand for medical care.
