The chapter focuses on how different cutting-edge techniques can be used to study electrophysiological, pathomorphological, and biochemical changes in the ,Äúepileptic focus,Äù area of the cerebral cortex and white matter to see how epileptic seizures become drug-resistant and how it affects the other regions of the brain. The authors highlight the significance of neuroinflammation and apoptosis in the epilepsy pathogenesis providing EEG characteristics and describing structural changes in the cortex and white matter under such conditions as focal cortical dysplasia and epileptic leukoencephalopathy. Particular focus is given to structural and functional changes in the hippocampus and the role of hippocampal sclerosis in epilepsy. Key conceptions regarding the epileptic focus formation are outlined.

This volume of International Review of Neurobiology concentrates on modern concepts of focal epileptic networks. The volume addresses specific topics such as seizures (including transition and termination), limbic networks, alteration of metabolism, and neocortical focus and malformation of cortical development, among others. Published since 1959, International Review of Neurobiology is a well-known series appealing to neuroscientists, clinicians, psychologists, physiologists, and pharmacologists. Led by an internationally renowned editorial board, this important serial publishes both eclectic volumes made up of timely reviews, and thematic volumes that focus on recent progress in a specific area of neurobiology research. Our knowledge about the mechanisms involved in pathophysiology of epilepsy has rapidly expanded during last decade This special volume brings overview about modern concepts of epileptic focus organization and about the altered neural network dynamics which results in propensity of the brain tissue to generate spontaneous and repeated seizures

No other neurological condition allows the same opportunities for an intracranial electrophysiological study of the human brain as epilepsy does. Epileptic surgery is designed to remove the epileptic focus from the human brain, thereby effecting either cure or substantial reduction of seizures in an individual with an otherwise intractable condition. Its use as a treatment modality dates from the late 19th century, and it has become a widely used treatment option throughout the world in the last 20-30 years. The complexity of epilepsy cases in surgical centres, and the need for invasive electrode studies for pre-surgical evaluation, are both greatly increasing. Invasive Studies of the Human Epileptic Brain is the definitive reference text on the use of invasive electroencephalographic (EEG) diagnostic studies in human epilepsy. Written by some of the most renowned epilepsy experts of the 20th and 21st centuries, the authors provide their expertise and insights into the identification and mapping of intracranial epileptiform and non-epileptiform activity, mapping of the human brain function, and approaches in the use of invasive electroencephalography in a variety of clinical situations. The book is organized into an easily readable series of chapters and is brilliantly illustrated with case studies; each providing an intuitively comprehensive approach to invasive brain studies.

An important area of current research in epilepsy focuses on identifying the specific regions within the brain that are affected in individuals with recurring seizures. The epileptogenic process may result not only in pathology in focal cortical regions, but abnormalities in subcortical structures, such as thalamus and basal ganglia, and in intercortical and intracortical connecting white matter pathways. Novel methods of treating refractory epilepsy are urgently needed. The goal of identifying for each affected individual the specific brain regions that are involved offers the promise that novel methods of treatment will one day be developed that specifically target those abnormal regions. Researchers from disparate fields are required to develop and advance this area of research, and this current topic proposes to place a spotlight on the “state of the art” of methods to identify the abnormal networks. Recent work covering a wide variety of disciplines and technologies, including dense array electroencephalography (dEEG), novel methods of analyses of both the interictal dEEG and intracranial EEG (icEEG), magnetoencephalography (MEG), high-resolution magnetic resonance imaging (MRI), functional MRI (fMRI), simultaneous fMRI-EEG, fMRI connectivity measures, simultaneous dEEG-icEEG, and techniques to coregister patient-specific MRI (including white matter pathways) and dEEG, are all examples of areas of research that have contributed to a greater understanding of potential epileptogenic regions. We asked for individuals with expertise in an area of research that expands an understanding of identifying epileptic networks to contribute to this research topic.

There are numerous books dealing with epilepsy but none of them is entirely devoted to the manifold relationships between language and epilepsy. The present monograph focuses on the verbal shortcomings, disorders, deviances, and peculiarities which may be observed in patients with epilepsy, and endeavors to uncover the links between the observed linguistic features and the disturbance of bio-electrical activity that characterizes epilepsy. It describes the many verbal impairments and involuntary verbal behaviours which may occur during epileptic seizures, depicts the various types of language-induced reflex epilepsy, discusses the possible relations between stuttering and epilepsy and between literary production and epilepsy, analyses the verbal consequences of surgery for intractable epilepsy, and deals extensively with the durable verbal deficits which may be due to, or be concomitant with, some forms of abnormal bio-electrical brain activity. The book is meant primarily for clinicians and caretakers who are professionally concerned with epilepsy. However, it is written in such a way that it can be easily understood by lay people who would like to gain an insight into the nature of epilepsy and into the verbal phenomena which may accompany it.
