The Leukemias: Etiology, Pathophysiology, and Treatment documents the proceedings of an International Symposium on The Leukemias: Etiology and Pathophysiology, held at Henry Ford Hospital, Detroit, Michigan, 8-10 March 1956. This volume gives the present complex position of leukemic etiology and pathophysiologic disturbance, a much needed knowledge of detailed leukocytic functions and comprehensive thought on the therapy of the leukemias. Dynamic Morphology is the attempt to correlate surface architec ture and shape of fixed cells, as visualized by scanning electron microscopy (SEM), with the behavior of living cells, recorded by microcinematography (MCM). If SEM and MCM are used concurrently for the analysis of cell populations, a dynamic inter pretation of SEM photographs is only valid if the experimental conditions are identical for the two techniques. This is achieved by allowing the cells to settle on a glass surface where they remain long enough to perform their various activities under conditions identical for both techniques (for technical details see Methodology). The analysis of a population necessitates the study of a large number of cells. This prerequisite is met by operating the scanning electron micro scope at low levels of magnification, and by using culture chambers for cinematography. It can be argued that the examina tion of attached cells excludes a complete SEM survey of a population, as cells not adhering from the outset or becoming detached during the different preparatory steps are lost. For this, cinematography proved to be a reliable control: All cell types recognized in time-lapse films were also seen in scanning electron (SE) micrographs. Another, and more general, objection to a dynamic interpretation concerns the artificiality of cellular behavior on glass. This is true, but does not invalidate compara tive studies making use of this substrate.

The objective of the treatment of acute leukemia involves the eradication of all neoplastic cells, including the last one. Ideally, treatment should be controlled by monitoring cell kill. If the last cells could be discovered and their biological properties be determined, the qualitative and quantitative effects of treatment should be directly evaluable. This should ultimately permit a calculated tumor cell reduction thereby avoiding overtreatment and excessive toxicity and thus providing a basis for individualized antileukemic treatment. In recent years several new developments have contributed to the selective discovery of minimal numbers of leukemic cells which are hidden among the normal cells in the marrow cavities. These methods are the first steps to the realization of the therapeutic goals indicated above. They include the production and ap plication of monoclonal antibodies against differentiation antigens on the cell sur face, the use of pulse cytophotometry - and cell sorter techniques, the employment of cytogenetics, the development of culture techniques for selective growth of precursor cells and several others. These methodologies offer prospects for refined diagnosis and, as far as the elimination of leukemic cells is concerned, the further development of autologous bone marrow transplantation. Eliminating tumor cells from autologous grafts requires the detailed knowledge of the cellular inter relationships within the neoplasm so that the neoplastic cells responsible for tumor propagation are specifically removed. Recognition and characterization of the clonogenic cells of the neoplasm should then lead to determining their sensitivity to the therapeutic agents which are clinically applied.

Much of the attention presently paid to leukemia is the result of recent progress in understanding and treatment. Chemotherapy of leukemia started in the late 1940s, and combination therapy evolved in the late 1950s. It was at that time that the clonality of leukemia was realized, after the discovery of chromosomal and then biochemical and immunological markers. And now we have the new data on retroviral and cellular oncogenes and the reports on human T -cell lymphoma/ leukemia viruses. Many more steps forward could be enumerated in a field which is so rapidly making the hematological textbooks outdated. In this volume, thirteen in-depth reviews from large multicenter trials in the Federal Republic of Germany summarize the current state of diagnosis and management of leukemias. Childhood ALL and AML adult ALL and AUL were investigated. While cure appears to be achievable for more and more patients with acute leukemia, we still pursue the aim of optimal palliation in chronic leukemias. The management of patients with leukemia therefore varies to a large extent in aggressiveness of therapy.
