The only reference wholly dedicated to this complex topic, Diagnosis and Treatment of Mitral Valve Disease: A Multidisciplinary Approach provides authoritative, must-have information on the disease processes affecting the mitral valve, interpretation of diagnostic studies, and medical, transcatheter, and surgical treatments needed for mitral valve repair. Using a multidisciplinary approach, Drs. Scott Goldman and William Gray oversee a “who’s who team of contributing authors who offer a holistic view of the modern treatment of mitral valve disease for today’s cardiac surgeons, cardiologists, and other cardiac team members. Includes foundational chapters on anatomy and pathophysiology of the mitral valve—critically important topics when treating multiple complex and highly dynamic structures that function as a single unit. Covers diagnostic tools, medical treatments, surgical therapies, and transcatheter therapies, and provides tips for developing a successful mitral valve program (with baseline thresholds, common pitfalls, and a checklist for hospitals). Uses an accessible, quick-reference format that features takeaway chapter openers, bulleted key points, end-of-chapter summaries, a common nomenclature and glossary of terms, key references, and ample line drawings, photographs, tables, charts, and graphs throughout. Incorporates the essential role of imaging in structural transcatheter interventions, including echocardiography and transcatheter edge-to-edge repair (TEER), and their uses in diagnostic work-up, procedural planning, intra-procedural guidance, and follow-up. Features dozens of procedural videos, including transthoracic echocardiogram; cardiac MRI for mitral regurgitation; transcatheter mitral valve repair; transcatheter valvuloplasty; percutaneous mitral valve interventions; MitraClipTM positioning; and many more.

This book provides a comprehensive review of the basic science of the mitral valve needed to begin to comprehend both normal and the failing valve. This knowledge is infrequently discussed in the clinical arena, with a rush towards definition of lesions based on limited analysis. It provides a ready single source of the most important basic science subjects pertaining to the valve, and the pathologies found therein, and thus a more complete understanding of the factors that result in dysfunction may be appreciated. Mitral Valve Disease: Basic Sciences and Current Approaches to Management includes chapters on embryology and anatomy, including details of structural, comparative and functional features. Since the valve is completely influenced by genetics and molecular biology, subjects rarely if ever addressed in the clinical arena, these topics receive in-depth attention. Coverage concludes with lengthy discussions of the importance of multi-disciplinary work, along with the importance of the electrophysiological disturbances encountered in some groups of these patients. It therefore is an essential and up-to-date resource for the practicing and trainee cardiac surgeon and any researcher working within this topic.

This full-color atlas with accompanying video DVD provides a complete and practical "how-to" guide to planning and performing mitral valve repair surgery for mitral regurgitation. The book reviews the natural history of mitral regurgitation, the functional anatomy of the mitral valve, and the use of echocardiography in preoperative evaluation and surgical planning. Chapters describe and illustrate all techniques currently used for mitral valve repair and discuss results. A bound-in DVD presents narrated video clips of six cases that show the application of specific techniques. Each case begins with preoperative echocardiograms demonstrating the mitral valve defect and proceeds through key surgical maneuvers.

Surgical options for the treatment of degenerative, ischemic, inflammatory, and functional mitral valve diseases have expanded greatly in recent decades. Still, a dilemma exists with employing prosthetic mitral valves – in that mechanical valves exhibit good durability but require anticoagulation versus the unpredictable durability of biological valve prostheses. Over the past few years, it has become clear that reconstruction of the mitral valve with autologous tissues effectively addresses this dilemma, with no need for anticoagulation and excellent durability, combining the best qualities of both prosthetic options without the disadvantages. Moreover, newer repair strategies, such as autologous pericardial leaflet augmentation and artificial choral replacement have expanded reparative approaches into virtually all pathologies, with excellent long-term results as compared to prosthetic valve replacement. At present, mitral repair is becoming the dominant procedure for treatment of virtually all patients with mitral valve disease. This book is designed to illustrate contemporary and sometimes diverse surgical techniques for various types of mitral valve repair with contributions from many of the leaders in the field. Mitral Valve Repair and the recently published Aortic Root Surgery are book publications arising from the Berlin Heart Valve Symposium 2008 entitled “ The Biological Solution”. The Mitral Valve Repair session was postponed to June 2009 and was featured as the Berlin Mitral Valve Symposium at the joint meeting of the Society for Heart Valve Diseases and the Heart Valve Society of America in Berlin. The book contains a collection of proceedings on current techniques and outcomes of mitral valve repair by many of the experts in this field which provide an authoritative reference source for cardiac surgeons, family practitioners and clinicians.

Recent Progress in Mitral Valve Disease presents a comprehensive examination of the proceedings that resulted from the International Symposium that transpired in Paris. It discusses the anatomy and physiology of the mitral valve. It addresses the cyclical motion of the mitral valve leaflets. Some of the topics covered in the book are the cyclical motion of the mitral annulus; analysis of hypertrophic cardiomyopathy; diagnoses of acute myocardial ischemia; characteristics of an abnormally functioning, structurally abnormal mitral valve; atrial function in normal cardiac cycle; and atrial function in acute mitral regurgitation. The factors that modify regurgitant volume are fully covered. An in-depth account of the methods for animal preparation and dynamics of acute experimental mitral regurgitation are provided. The uses and limitations of echocardiography in the examination of the mitral valve are completely presented. A chapter is devoted to the timing of surgical intervention in chronic mitral regurgitation. Another section focuses on the status of prosthetic valves in the mitral position. The book can provide useful information to doctors, cardiologists, students, and researchers.

Mitral stenosis is due to thickening and immobility of the mitral valve leaflets, resulting in obstruction in blood flow from the left atrium to the left ventricle. The mitral valve leaflets are thickened and the commissures are fused along with thickening and shortening of the chordae tendineae. This mechanical obstruction leads to increases in pressure within the left atrium, pulmonary vasculature, and right side of the heart; in comparison, the left ventricle is unaffected if there is pure mitral stenosis.

The field of structural heart disease interventions is experiencing a stage of rapid growth with the development of percutaneous mitral valve repair therapies and percutaneous aortic valve replacement therapies. The MitraClip represents a new therapeutic frontier for mitral regurgitation and is expected to revolutionize repair of the mitral valve. For the first time in the world, physicians now have the capability to repair the mitral valve percutaneously without the need for a sternotomy or cardiopulmonary bypass. However, as with all new endeavors, there is a steep learning curve, not just for the individual operator but also for the entire field. The MitraClip technology and technique is technically complex and is guided by echocardiography and fluoroscopy. It is an imaging intensive procedure that requires deep 3-dimensional understanding of normal and abnormal cardiac anatomy. Not all mitral regurgitation cases are suitable and not all cases are straightforward. It is therefore crucially important for physicians in this specialty to communicate their experience, successes, failures and complications with each other. Written by the leaders in this breakthrough field, the Atlas of Percutaneous Edge-to-Edge Mitral Valve Repair is the first of its kind for the new percutaneous MitraClip procedure. The Editors have designed the book to be a comprehensive reference manual and to provide cardiologists with true examples of basic and complex cases of percutaneous mitral valve repair using the technology. In addition, this book provides step-by-step guides to the procedure and complications as well as steps to recover from them are discussed. The Atlas is anticipated to be a very valuable reference for both novice and experienced interventional cardiologists and echocardiologists. As percutaneous valve repair becomes more common it will also be of great interest to general cardiologists and surgeons.
