Impacts that threaten or indeed compromise the integrity of tissues trigger the development of a defence response, which through the activity of the nociceptive system includes pain. If the noxious impact does not induce tissue damage, the pain, called “nociceptive” pain, ceases within seconds after the impact is withdrawn. In contrast, if tissue damage does occur, a pain experience that usually persists until the injury is resolved and includes two major pathological sensory experiences, hypersensitivity to heat stimuli (i.e. heat hyperalgesia) and/or hypersensitivity to mechanical stimuli (i.e. mechanical allodynia) develop. The cellular and molecular mechanisms underlying the development of nociceptive pain are fairly well understood. Our understanding of the development of pain associated with tissue injury has also significantly improved in the last decades. Hence, two fundamental mechanisms, interactions between the nervous and immune systems both within and without the central nervous system and sensitization that is a use-dependent increase in the sensitivity and activity of neurons involved in nociceptive processing have been identified being pivotal for the development of tissue injury-associated pain. However, important details of the cellular and molecular mechanisms, which account for the development of the pathological sensory experiences and those experiences becoming persistent, still await elucidation.

As we stated in our message in the book of abstracts for this congress, we have planned the programme over a long period with one clear objective: to present musculoskeletal medicine as an integral part of orthodox medical practice, rather than as something alternative or complementary. To this end we have based the plenary programme as far as possible on accepted epidemiological, anatomical, physiological and pathological phenomena. Scientifically well-validated material must surely be the base upon which any viable musculoskeletal medicine practice may be built. While we have chosen the plenary programme to reflect musculoskeletal medicine as a part of orthodoxy, we realize and wish to emphasize that there is a wealth of original work that has been carried out within FIMM. For this reason our first innovation for the congress was to invite members of the scientific advisory committee to select for a 'directed' programme the three topics they felt were of greatest current importance. The results of this democratic procedure was the choice of the sacroiliac joint, a comparison of manual therapies and biomechanics. This illustrates the broad direction of present thinking within FIMM.

Pain is a subject of significant scientific and clinical interest. This has resulted both from realistic rodent models, and the publication of imaging, psychological and pharmacological studies in humans. Investigators studying rodents refer to anatomical and physiological studies in non-human primates to make their results relevant to humans. Psychophysical and pharmacological studies in humans are interpreted in terms of anatomical and physiological studies in animals; primarily evidence from rodents and cats. There are significant differences in pain mechanisms between these species and primates. Over 20 years of imaging studies have demonstrated the activation of human cortical and subcortical structures in response to painful stimuli. Interpretation of these results relies upon an understanding of the anatomy and physiology of these structures in primates. Jones, Lenz, Casey and Willis review the anatomy and physiology of nociception in monkeys and humans, and provide a firm basis for interpreting studies in humans.

The study of the functional organization of the first synapse of the centripetal visual pathway at the outer plexiform layer level (OPL) ought to be made through the application of combined histological, electrophysiological, and neurochemical techniques. A large amount of new evidence has been accumu lated in the past 20 years on the structure of the retina and on the electrical responses of retinal cells to light stimulus. Also, recently, many substances considered as neurotransmitters in the brain have been found in the retina. The goal of the study of retinal function is to integrate the data obtained by structural and electrophysiological techniques and to identify and determine the role played by neurotransmitters or neuromodulators in the function of the retina. In this study it is important to realize the morphological and biochemical diversi ty displayed by the visual cells in the vertebrate retina which, according to Cresci telli (1972), has been produced "through the interaction of natural selection with diversity in the photic environment." The evidence obtained shows that bipolar and especially horizontal cells, closely related to visual cells, display morphologi cal and probably biochemical differences among classes, genus, and even species according to the photic environment. These differences give peculiarities to the organization of the OPL, which must be taken into account when studying a par ticular retina with electrophysiological or neurochemical techniques.
