This book proposes the importance of new systems of drug design and delivery based on cancer pathophysiology in addition to cancer molecular and cellular biology. The current studies based on molecular and cellular biology while ignoring pathophysiology and pharmacology may be leading the development of antitumor drugs in the wrong direction and wasting a lot of money. Although there have been numerous reports of genetic and phenotypic changes in tumors, a large body of pathological and clinical evidence supports the conclusion that there are no pivotal changes in tumor cells that distinguish them consistently and reliably from normal dividing cells. Unlike using antibiotics against bacterial infection, therefore, anticancer agents (ACAs) need to be delivered selectively to tumor tissues and should be kept there long enough to reproduce the concentrations they reach in the Petri dish, which is a closed space where the cytocidal effects of any anticancer agents (ACAs) including molecular targeting agents are very strong. In the body, however, administered ACAs are cleared with the passage of time. Furthermore, most human cancers possess abundant stroma that hinders the penetration of drugs into the tumor microenvironment. Therefore, to overcome these difficulties, novel drug delivery systems have been designed, such as nanoparticles and ACA conjugated antibodies to stromal components and to cancer cell surface antigens. These advances are described in this book after the first section, which describes core features of the pathophysiology of the cancer microenvironment, on which these new developments are based.

"Breast cancer is the most common cancer diagnosed among women and the emergence of metastatic breast cancer, the most advanced stage of the disease, is largely incurable. Upon dissemination from the primary tumor, breast cancer spreads primarily to the bone, lung, liver and brain. The liver represents a site of metastasis with poor prognosis and low survival rates. Despite its clinical relevance, little is known regarding the cellular and molecular mechanisms promoting breast cancer metastasis to the liver. We have recently identified that neutrophils, which account for the majority of circulating leukocytes and are typically the first responders at the site of inflammation, have been implicated in promoting breast cancer liver metastasis. The work presented in this thesis is focused on identifying the role of pro-metastatic neutrophils and how they are generated, recruited to the tumor microenvironment and promote the growth of liver metastases. Using well-characterized breast cancer cell lines with different metastatic potential to the lung or the liver, we identified G-CSF, as an important cytokine that is required, but not sufficient, to promote the mobilization of immature low density neutrophils (LDNs) in breast cancer liver metastasis. We show that circulating immature LDNs promote the growth of liver metastasis whereas mature high-density neutrophils (HDNs) were anti-metastatic. Additionally, immature LDNs display increased metabolic flexibility, which facilitates their ability to undergo NETosis (neutrophil extracellular traps) under glucose-deprivation and contribute to the establishment and growth of liver metastases.To investigate the ability of the neutrophil subpopulations to migrate to the liver microenvironment, we utilized both in vitro and in vivo experiments to demonstrate that immature LDNs preferentially infiltrate the liver microenvironment due to tumor-derived chemoattractants. Using shRNA-mediated approaches and pharmacological inhibitors, we show that C3a is a novel chemoattractant that preferentially promotes the recruitment of immature LDNs, with no effect on HDN recruitment.Finally, to determine the role of neutrophils in mediating the growth of liver metastases, we used various immunocompromised mice to determine whether additional immune cells are impacted by pro-metastatic neutrophil functions. We show that immature low-density neutrophils mediate immune suppression of NK-cells, but not T-cells, through the neutrophil-specific expression of CD155. CD155 interacts with an inhibitory receptor, TIGIT, expressed on NK cells, thereby inhibiting NK-mediated tumor cell clearance.Altogether, the body of thesis demonstrates that immature LDNs, a subset of neutrophils that are enriched in mice and humans during cancer progression, are pro-metastatic and have unique mechanisms to migrate to the tumor microenvironment and promote immune suppression"--
