Food safety hazards are increasingly being recognized as a major public health problem worldwide, yet among developing countries, there is limited understanding of the wider-ranging socio-economic costs of unsafe food and the benefits of remedial or preventative measures. This limited evidence base has led many countries to underinvest in food safety, or invest inefficiently in reaction to serious outbreaks of foodboroe illness, other food scares, or trade interruptions. For many countries experiencing rapid urbanization and dietary changes, the growing complexity of food safety hazards is outpacing if not overwhelming prevailing food safety management capacity-both in government and in supply chains. This report strengthens the economic case for increased public investment and other policy attention on food safety in developing countries. [t is directed primarily at policy-makers, although researchers, development practitioners and food safety specialists will also find its content of value. By synthesizing and interpreting the available evidence on the economic costs of unsafe food in relation to both domestic markets and trade, the report positions food safety as an integral part of economic development and food system modernization. It goes on to provide guidance on ways in which public policy and investment can improve food safety awareness and behavior from farm to fork.

Ensuring the safety of food and the quality and safety of medicines in a country is an important role of government, made more complicated by global manufacturing and international trade. By recent estimates, unsafe food kills over 400,000 people a year, a third of them children under 5, mostly in low- and middle-income countries; every year poor quality medicines cause about 70,000 excess deaths from childhood pneumonia and roughly 8,500 to 20,000 malaria deaths in sub-Saharan Africa alone. The Federal Drug Administration (FDA) Office of Global Policy and Strategy is charged with improving capacity of the agency's foreign counterpart offices and increasing understanding of the importance of regulatory systems for public health, development, and trade. At the request of the FDA, this study sets out a strategy to support good quality, wholesome food and safe, effective medical products around the world. Its goal is to build on the momentum for strengthening regulatory systems and to set a course for sustainability and continued progress. The 2012 report Ensuring Safe Food and Medical Products Through Stronger Regulatory Systems Abroad outlined strategies to secure international supply chains, emphasized capacity building and support for surveillance in low- and middle-income countries, and explored ways to facilitate work sharing among food and medical product regulatory agencies. This new study assess progress made and the current regulatory landscape.

The new WHO Global Strategy for Food Safety 2022-2030 was adopted by the Seventy-fifth World Health Assembly in 2022. Foodborne diseases (FBD) are an important externality of agriculture and food systems, but only recently have they risen up the development agenda as the result of growing awareness of the health and economic burdens of FBD and how they relate to food systems with a focus on low-income and middle-income countries (LMIC), particularly African nations. The health burden of FBD is comparable with that of malaria, and over 90% falls on people in LMIC, with an economic burden of more than US$100 million per year. FBD have many other, less well-estimated effects on nutrition, gender, equity, and the environment. While understanding of food safety in domestic markets of LMICs has advanced greatly, risk management is in its infancy. This Research Topic will bring together leading regional perspectives on food safety in LMICs.
