As is the case in all fields of medicine, developmental endocrinology is now being studied at the molecular level. In this volume world-class researchers review the advances of the past decade in the study of normal and abnormal organogenesis of the thyroid gland and of the ontogeny of its function. They describe human thyroid development and its defects with the help of genetic studies in mouse models. Genetic defects of thyroid hormone synthesis are covered and their clinical relevance debated. The important field of thyroid cancer in the context of spontaneous occurrence and as part of familial neoplasia syndromes is described in detail. Finally, the important problem of environmental iodine deficiency which has emerged as a global public health concern is addressed. For the first time, a decade of work is presented in a concise and highly readable form. Offering valuable insight both for senior clinicians and graduate students, this publication will be of central interest to basic scientists involved in developmental biology as well as to pediatricians and endocrinologists dealing with patients with congenital disorders of thyroid function.

Thyroid gland is one of the largest endocrine gland in the body. It possesses one of the richest blood flows to that of adrenal gland and kidney. Morphological and morphometric parameters of thyroid in general and fetal thyroid Histogenesis were discussed and studied. Few studies on developmental anatomy were available in the literature. Histogenesis of thyroid glands revealed that, there was a delay in the pre colloid formation to follicular growth stage. Disorders of thyroid gland development and function are relatively common affecting approximately one newborn infant in 2000-4000. The present study helps that delay in the pre colloid stage to follicular growth stage of thyroid gland leads to anomalies. Dysfunction and anatomic abnormalities of the thyroid are among the most common diseases of the endocrine glands.

Thyroid hormone plays an important role in development and functional maintenance in the central nervous system. Deficiency of thyroid hormone during fetal and early postnatal life induces abnormal development known as cretinism in humans. However, the molecular mechanism of thyroid hormone action has not yet been fully understood. Thyroid hormone action in the brain may be disrupted under various pathological conditions. In addition, environmental factors including endocrine-disrupting chemicals and bacterial endotoxins may disrupt thyroid hormone action in brain, causing abnormal brain development and functional disruption. This is a first book to comprehensively describe the effect of thyroid hormone disruption in the central nervous system. The first section deals with the disruption of thyroid hormone action at the molecular level. First the authors provide a summary of the possible molecular mechanisms of thyroid hormone action in the brain, then they discuss several factors that may disrupt thyroid hormone action. In the second section, animal models to study thyroid hormone action will be introduced. An interesting character of thyroid hormone deficiency is that, without thyroid hormone, the thyroid hormone receptor may act as a “repressor” of gene expression, causing more severe consequence than those of thyroid hormone receptor knockout animals. Thus, several different kind of animal models may be used to clarify the role of thyroid hormone and its receptor in the brain. In the third section, human studies on thyroid disease and neurodevelopment will be introduced. Although endemic cretinism induced by iodine deficiency and sporadic cretinism by various thyroid mutation are well known, the pathophysiological mechanisms that create each abnormal phenotype are not fully understood.

During the 1950s, with the Cold War looming, military planners sought to know more about how to keep fighting forces fit and capable in the harsh Alaskan environment. In 1956 and 1957, the U.S. Air Force's former Arctic Aeromedical Laboratory conducted a study of the role of the thyroid in human acclimatization to cold. To measure thyroid function under various conditions, the researchers administered a radioactive medical trace, Iodine-131, to Alaska Natives and white military personnel; based on the study results, the researchers determined that the thyroid did not play a significant role in human acclimatization to cold. When this study of thyroid function was revisited at a 1993 conference on the Cold War legacy in the Arctic, serious questions were raised about the appropriateness of the activityâ€"whether it posed risks to the people involved and whether the research had been conducted within the bounds of accepted guidelines for research using human participants. In particular, there was concern over the relatively large proportion of Alaska Natives used as subjects and whether they understood the nature of the study. This book evaluates the research in detail, looking at both the possible health effects of Iodine-131 administration in humans and the ethics of human subjects research. This book presents conclusions and recommendations and is a significant addition to the nation's current reevaluation of human radiation experiments conducted during the Cold War.

This book covers the entire spectrum of thyroid diseases in childhood, focusing on the recent advances that have been achieved, from progress in basic science research through to novel or improved approaches to diagnosis and treatment. Introductory chapters discuss thyroid embryogenesis and the role of thyroid hormones in fetal development. The two contrasting forms of thyroid dysfunction, hypo- and hyperthyroidism, are then considered in depth, with particular attention to the molecular causes of congenital hypothyroidism. Among the other topics addressed are autoimmune thyroiditis, thyroid nodules, and pediatric neoplasms. The book concludes with an overview of promising therapeutic approaches, such as stem cell therapy. Each topic is treated by an eminent expert in the field, ensuring consistently high quality. Thyroid Diseases in Childhood will be an important source of information for endocrinologists, pediatricians, oncologists, and gynecologists, as well as other professionals interested in this topic.

When the Medicare program was established in 1965, it was viewed as a form of financial protection for the elderly against catastrophic medical expenses, primarily those related to hospitalization for unexpected illnesses. The first expansions to the program increased the eligible population from the retired to the disabled and to persons receiving chronic renal dialysis. It was not until 1980 that an expansion of services beyond those required "for the diagnosis or treatment of illness or injury or to improve the functioning of a malformed body member" was included in Medicare. These services, known as preventive services, are intended either to prevent disease (by vaccination) or to detect disease (by diagnostic test) before the symptoms of illness appear. A Committee was formed "to conduct a study on the addition of coverage of routine thyroid screening using a thyroid stimulating hormone test as a preventive benefit provided to Medicare beneficiaries under Title XVIII of the Social Security Act for some or all Medicare beneficiaries."
